Streszczenie Wstęp: Rak stercza jest najczęściej rozpoznawanym nowotworem litym wśród mężczyzn z wykrywalnością na poziomie 214 przypadków na 100 000 mieszkańców. Jest jednocześnie drugą co do częstości przyczyną zgonów pośród chorób nowotworowych mężczyzn w krajach europejskich. Obecnie standardem diagnostycznym w diagnostyce raka stercza jest biopsja systematyczna gruczołu krokowego. Z powodu niedoskonałości dostępnych badań obrazowych rak stercza jest jednym z niewielu nowotworów litych, w których diagnostyka opiera się na ocenie losowo pobranych wycinków według zaplanowanego schematu. Od wielu lat trwają więc badania nad opracowaniem metody obrazowania pozwalającej na bardziej precyzyjną lokalizację zmian podejrzanych w sterczu, dzięki której nastąpiłaby redukcja liczby pobieranych podczas biopsji rdzeni tkankowych, a co za tym idzie zmniejszenie dyskomfortu chorego, liczby powikłań, a także kosztów związanych z diagnostyką. Jedną z takich metod jest elastografia, opracowana na początku lat 90. ubiegłego wieku. Metoda ta polega na ocenie ściśliwości poszczególnych obszarów gruczołu krokowego i na podstawie różnic w ściśliwości/twardości precyzyjnym wyznaczeniu miejsc podejrzanych o przemianę nowotworową. Cele pracy: W pracy zamierzano dokonać oceny zastosowania elastografii ultrasonograficznej we współczesnej diagnostyce raka stercza, a także zaprezentować i porównać dwa rodzaje elastografii najczęściej wykorzystywane w diagnostyce nowotworów gruczołu krokowego: elastografię odkształceń względnych (ES) i elastografię fali poprzecznej (SWE). Kolejnym celem pracy było porównanie obu prezentowanych metod diagnostycznych z biopsją systematyczną jako obowiązującym standardem diagnostycznym. Zamierzano także określić zalety i ograniczenia metod elastograficznych na etapie diagnostyki raka stercza oraz wyznaczyć potencjalne miejsce elastografii w obowiązującym standardzie diagnostycznym. Materiał i metoda: Do badania włączono 160 mężczyzn z podejrzeniem raka stercza na podstawie podwyższonego poziomu swoistego antygenu sterczowego (PSA), nieprawidłowości w palpacyjnej ocenie gruczołu krokowego w badaniu przezodbytniczym (DRE) lub obu tych powodów jednocześnie. Z różnych przyczyn, zależnych od pacjentów (brak zgody na biopsję, przeciwwskazania do wykonania procedury inwazyjnej) i błędów technicznych czy proceduralnych, z badania wyłączono 14 osób. Ostatecznie pełnej ocenie na wszystkich etapach diagnostycznych poddano 146 mężczyzn rasy kaukaskiej w wieku od 46 do 85 lat (średnio 66,9 roku). Poziom PSA kształtował się w tej grupie w przedziale od 1,02 do 885 ng/ml (średnio 21,58 ng/ml), gęstość PSA (PSAD) w zakresie od 0,024 do 16,389
(średnio 0,456). Objętość gruczołu krokowego w ocenie ultrasonograficznej w badaniu przezodbytniczym (TRUS) wyniosła od 17 do 149 ml (średnio 53,8 ml). W najliczniejszej, 90-osobowej grupie (61,6%) badania zalecono na podstawie podwyższonego poziomu PSA, u 39 osób (26,7%) wskazaniem do podjęcia diagnostyki był łącznie podwyższony poziom PSA i nieprawidłowości w badaniu przezodbytniczym, a u 17 osób (11,64%) odstępstwa od normy w DRE. Badanie przeprowadzono pomiędzy 14 maja 2011 roku a 20 września 2014 roku w Zakładzie Diagnostyki Obrazowej II Wydziału Lekarskiego Warszawskiego Uniwersytetu Medycznego i Oddziale Urologii i Onkologii Urologicznej im. Prof. Tadeusza Krzeskiego Mazowieckiego Szpitala Wojewódzkiego w Siedlcach. Chorych podzielono na dwie grupy pierwsza, 102-osobowa grupa poddana została ocenie elastograficznej za pomocą ES. W tej grupie w przypadku stwierdzenia miejsc podejrzanych o przemianę nowotworową w badaniu elastograficznym wyznaczano stosunek ściśliwości, SR (Strain Ratio), pomiędzy obszarem zainteresowania a symetrycznym obszarem stercza. Obszary o SR większym od 2 kwalifikowano do biopsji celowanej pod kontrolą TRUS. Druga, 44-osobowa grupa chorych poddana została ocenie elastograficznej za pomocą SWE. W tej grupie do biopsji celowanej kwalifikowano wszystkie ujawnione podczas oceny elastograficznej zmiany o twardości maksymalnej przekraczającej 50 kpa. Wszyscy chorzy z pierwszej i drugiej grupy, niezależnie od wskazań elastografii, zostali poddani biopsji systematycznej 8 lub 10-rdzeniowej będącej punktem odniesienia i obowiązującym obecnie standardem diagnostycznym. Ocena elastograficzna, biopsje celowane i systematyczne wykonywane były przez lekarzy posiadających co najmniej pięcioletnie doświadczenie w diagnostyce ultrasonograficznej gruczołu krokowego. Wyniki: W pierwszej grupie łącznie na podstawie biopsji celowanych pod kontrolą ES i biopsji systematycznych rozpoznano 52 zmiany nowotworowe. Biopsja celowana ujawniła 42 zmiany nowotworowe. Podczas biopsji celowanych pobrano 230 rdzeni tkankowych, z których 78 (33,9%) wykazało przemianę nowotworową. Na potrzeby biopsji systematycznej w tej samej grupie chorych pobrano 1002 rdzenie tkankowe. Czułość biopsji celowanej pod kontrolą ES w grupie wszystkich stwierdzonych nowotworów oceniono na 80,8%. Biorąc pod uwagę rozpoznanie raków istotnych klinicznie według kryteriów Epsteina, czułość ES była wyższa i wyniosła 85,7%. Czułość biopsji celowanej pod kontrolą ES wzrastała wraz ze stopniem złośliwości nowotworu wyrażonym w skali Gleasona. Dla zmian ocenionych na 6 punktów w skali Gleasona czułość wyniosła 44,4%, na 7 punktów 78,9%, na 8 punktów 94,1%, na 9-10 punktów 100%. Czułość biopsji systematycznej stanowiącej punkt
odniesienia w tej grupie chorych wyniosła 90,4%. Biopsja systematyczna nie ujawniła 5 zmian nowotworowych spośród 52 wykrytych w tej grupie. W drugiej grupie łącznie na podstawie biopsji celowanych pod kontrolą SWE i biopsji systematycznych rozpoznano 21 zmian nowotworowych. Na potrzeby biopsji celowanych w tej grupie pobrano 95 rdzeni tkankowych, z których 31 (32,6%) wykazało przemianę nowotworową. Na potrzeby biopsji systematycznej pobrano 440 rdzeni tkankowych. Czułość SWE w grupie wszystkich nowotworów oszacowano na 85,7%, zaś w grupie nowotworów istotnych klinicznie według kryteriów Epsteina na 90%. Zauważono ponadto korelację czułości SWE ze stopniem zaawansowania nowotworu wyrażonym w skali Gleasona. Dla 6 punktów w skali Gleasona czułość wyniosła 57%, natomiast dla 7, 8, 9 i 10 punktów 100%. Czułość biopsji systematycznej w grupie chorych poddanych ocenie przy użyciu SWE wyniosła 100%. Wnioski: Ocena elastograficzna wydaje się być cennym narzędziem w diagnostyce raka stercza na etapie zmierzającym do wykrycia choroby, pozwalając na redukcję liczby pobieranych rdzeni, a co za tym idzie powikłań i kosztów. W pierwszej grupie biopsja celowana pod kontrolą ES okazała się cennym uzupełnieniem biopsji systematycznej, podnosząc jej czułość o 9,6%. SWE przewyższa ES pod względem czułości (80,85 vs 85,75), ponadto jest metodą ilościową, mniej zależną od operatora i pozwalającą na porównawczą analizę wyników. Stwierdzono też, że jest to metoda o największej czułości w grupie raków stercza o stopniu złośliwości ocenionym w skali Gleasona na 7 w porównaniu nie tylko z ES, ale też biopsją systematyczną. Wydaje się, że SWE może zastąpić biopsję systematyczną jako standard w diagnostyce raka stercza. Celem ugruntowania pozycji elastografii jako narzędzia w diagnostyce tego nowotworu konieczne są dalsze badania. Summary Title: Ultrasound elastography assessment in the diagnosis of prostate adenocarcinoma Introduction: Prostate cancer is the most commonly diagnosed solid tumour in men, with a detection rate of 214 cases per 100,000 inhabitants. At the same time, it is the second leading cause of mortality due to neoplastic diseases among European men. Systematic biopsy of the prostate gland is currently the diagnostic standard in prostate cancer. Due to the imperfection of the available imaging techniques, prostate cancer is one of few solid tumours whose diagnosis is based on randomly sampled specimens, following a predetermined scheme. For many years there has been ongoing research to develop an imaging technique that would allow for a more precise location of suspicious prostate lesions, and thus a reduction in the number of biopsy tissue cores, which would, in turn, reduce patient s discomfort, the number of complications as well as the costs of diagnostics. Elastography, which was developed in the early 90s of the last century, is one of these methods. The technique involves the measurement of compressibility in different prostate regions and, based on the differences in compressibility/hardness, a precise determination of regions suspected of neoplastic transformation. Aim: The aim of the paper is to assess the use of ultrasonographic elastography in the contemporary diagnosis of prostate cancer as well as to present and compare two types of elastography most often used in the diagnosis of prostate cancers: strain elastography (SE) and shear wave elastography (SWE). Another aim was to compare both diagnostic methods with systematic biopsy as a current diagnostic standard. We also intended to determine the advantages and limitations of elastographic methods in prostate diagnosis as well as to determine the potential place of elastography in the current diagnostic standard. Material and methods: A total of 160 men with suspected prostate cancer based on elevated prostate-specific antigen values (PSA), abnormal DRE findings or both these reasons were included in the study. A total of 14 patients were excluded from the study for various patient-dependent reasons (lack of consent for biopsy, contraindications to invasive procedures) or technical or procedural errors. Ultimately, 146 Caucasian men aged between 46 and 85 years (mean age 66.9 years) were included in a full assessment at all diagnostic stages. The PSA levels ranged in this group between 1.02 and 885 ng/ml (mean levels 21.58 ng/ml); PSA density (PSAD) ranged between 0.024 and 16.389 (mean 0.456). The volume of the prostate measured by TRUS ranged between 17 and 149 ml (mean 53.8 ml). Elevated
PSA levels were the reason for diagnostic evaluation in most men (61.6%), i.e. a group of 90 patients; elevated PSA and abnormal transrectal findings were an indication for diagnosis in 39 (26.7%); abnormal DRE was the reason for diagnostic evaluation in 17 (11.64%) patients. The study was conducted between May 14, 2011 and September 20, 2014 in the Department of Diagnostic Imaging of the Second Faculty of Medicine at the Medical University of Warsaw as well as the Tadeusz Krzeski Department of Urology and Oncologic Urology of the Mazovia Regional Hospital in Siedlce. Patients were divided into two groups. Group one including 102 patients was assessed using SE. If areas suspected of neoplastic transformation were identified based on elastography in this group of patients, the strain ratio (SR) between the region of interest and a symmetrical prostate area was determined. Areas with SR higher than 2 were qualified for TRUS-guided targeted biopsy. The second group, which included 44 patients, was assessed using SWE. In this group of patients, all lesions revealed by elastography which had a maximum hardness exceeding 50 kpa were qualified for targeted biopsy. All patients in group 1 and 2, regardless of elastography indications, underwent systematic 8- or 10-core biopsy, which represented both, a reference point and a current diagnostic standard. The elastography assessment as well as targeted and systematic biopsy procedures were performed by physicians with at least 5 years of experience in diagnostic ultrasound of the prostate gland. Results: A total of 52 neoplastic lesions were identified in group 1 based on SE-guided targeted and systematic biopsy. Targeted biopsy revealed 42 neoplastic lesions. A total of 230 tissue cores, including 78 (33.9%) showing neoplastic transformation, were collected during targeted biopsy. A total of 1,002 tissue cores were collected for systematic biopsy in this group. The estimated sensitivity of SE-guided targeted biopsy in the group of all diagnosed cancers was 80.8%. Considering the diagnosis of clinically significant cancers according to Epstein s criteria, the sensitivity of SE was higher, i.e. 85.7%. The sensitivity of SE-guided targeted biopsy increased with the Gleason score. The sensitivity was 44.4% for lesions with Gleason score of 6; 78.9% for Gleason score of 7; 94.1% for Gleason score of 8 and 100% for Gleason score of 9-10. The sensitivity of systematic biopsy, which was used as a reference point in this group of patients, was 90.4%. Systematic biopsy failed to reveal 5 out of 52 neoplastic lesions in this group of patients. In group 2, a total of 21 neoplastic lesions were diagnosed based on SWE-guided targeted biopsy and systematic biopsy. A total of 95 tissue cores, including 31 (32.6%) showing neoplastic transformations, were collected for targeted biopsy in this group of patients. A total of 440 tissue cores were collected for systematic biopsy. The estimated
sensitivity of SWE was 85.7% in the group of all cancers and 90% in the group of clinically significant cancers according to Epstein s criteria. Furthermore, a correlation was found between SWE sensitivity and the grade of the tumour (the Gleason score). The sensitivity was 57% for Gleason score of 6 and 100% Gleason score of 7, 8, 9 and 10. The sensitivity of systematic biopsy in the group of patients assessed using SWE was 100%. Conclusions: Elastography appears to be a valuable tool in the diagnosis of prostate cancer at the stage aimed to detect the disease, thus allowing for a decreased number of biopsy cores and, consequently, reduced complications and costs. In the first group of patients, SE-guided targeted biopsy proved to be a valuable supplementation of systematic biopsy, increasing its sensitivity by 9.6%. SWE is superior to SE in terms of sensitivity (80.84 vs. 85.75) and is a quantitative method, less dependent on the operator and allowing for a comparative analysis of results. It was also found that this method shows the highest sensitivity in the group of prostate cancers with Gleason score 7 compared to both SE and systematic biopsy. It seems that SWE can replace systematic biopsy as a standard in the diagnosis of prostate cancer. Further research is needed to reinforce the position of elastography as a tool in the diagnosis of prostate cancer.
