Celem badań było poszukiwanie korelacji między lekoopornością enterokoków izolowanych z zakażeń od ludzi a wytwarzaniem przez nie sideroforów. Szczepy oporne na fluorochinolony - ciprofloksacynę i norfloksacynę syntetyzowały większe ilości sideroforów niż szczepy wrażliwe na te dwa chemioterapeutyki. Słowa kluczowe: Enterococcus, lekooporność, siderofory Introduction: Enterococci belong to the normal bacterial flora of the gastrointensinal tract of humans. Enterococci are regarded as harmless commensal, and are even believed to have probiotic characteristics. However, they can cause variety of infections, including endocarditis, bloodstream infections and urinary tract infections. During the past several decades, enterococci, and particularly Enterococcus faecalis and E. faecium, have been identified as an important cause of nosocomial infections. Enterococci are intrinsically resistant to a broad range of antimicrobials. Infection caused by resistant strains are difficult to treat. Iron is an essential element for bacteria, but is not easily available in host organisms. Enterococci are iron dependent bacteria. Competition for iron between the host and bacteria is an important factor determining the course of bacterial infections. A common strategy among bacteria living in iron-limited environments is the secretion of siderophores, which can bind poorly soluble iron and make it available to cells via active transport mechanisms. The aim of the presented study was to evaluate the correlation between antibiotic resistance and siderophore production of bacteria of the genus Enterococcus. Methods: The study included 55 bacterial strains from genus Enterococcus belonging to two species Enterococcus faecalis and E. faecium. Antimicrobial susceptibility tests were carried out using disc diffusion methods with guidelines of European Committee on Antimicrobial Susceptibility Testing (EUCAST). Total siderophore activity in the culture supernatants was measured using chrome azurol S. Hydroxamate siderophores were assayed using a chemical-specific assay. Results: Antibacterial susceptibility pattern reveals that E. faecium is more resistant than E. faecalis. A significant correlation was found between resistance to fluoroquinolnes and siderophores production. Ciprofloxacin- and norfloxacin-resistant enterococal strains produced siderophores in large quantity. Conclusions: One of the most common infections caused by enterococci are urinary tract infections. Fluoroquinolones are an important group of antimicrobial agents used in this type of infection. Fluoroquinolones resistance of enterococci associated with increased synthesis of siderophores result in the increased virulence that may decide on the severity of the infection and the effectiveness of the treatment. Występowanie przeciwciał dla pałeczek Francisella tularensis w poszczególnych podklasach IgG u osób z tularemią Serum immunoglobulin IgG subclass distribution of antibody responses to Francisella tularensis in patients with tularemia Waldemar Rastawicki, Natalia Rokosz-Chudziak, Natalia Wolaniuk Zakład Bakteriologii NIZP-PZH w Warszawie Określono częstość występowania i poziom przeciwciał podklas IgG1-IgG4 dla inaktywowanych, rozbitych ultradźwiękami pałeczek F. tularensis u osób z tularemią. Przeprowadzone badania wykazały, że w przebiegu tularemii wytwarzane są przeciwciała należące do podklasy IgG1, IgG2 i IgG3. Najczęściej i na najwyższym poziomie wykrywano przeciwciała podklasy IgG2. Nie wykryto na diagnostycznie znamiennym poziomie przeciwciał podklasy IgG4. Słowa kluczowe: przeciwciała podklasy IgG, Francisella tularensis, tularemia Introduction: The present study was aimed at determining the IgG subclass distribution against F. tularensis in patients with tularemia. Methods: The total number of 56 serum samples obtained from patients with serologically confirmed tularemia were tested by in-house ELISA with bacterial sonicate as the antigen for the presence of IgG1, IgG2, IgG3 and IgG4 antibodies to F. tularensis. Based on the results of determining the level of antibodies in the sera of 30 blood donors, the cut-off limit of serum antibodies for each subclass was set at arithmetic mean plus three standard deviations. Results: Antibodies of subclass IgG1 to F. tularensis were diagnosed in 41 (73.2%), IgG2 in 52 (92,9%) and IgG3 in 13 (23,2%) serum samples. The arithmetic mean of OD 450 of antibodies IgG2 was over three-times higher than antibodies IgG1 and IgG3 measured in all of tested serum samples. The concentration of IgG4 was below the detection level. Conclusions: In conclusion, IgG2 antibodies to F. tularensis are predominating IgG subclass in tularemia. This study showed also that subclasses of IgG1 and IgG3 but not IgG4 antibodies to F. tularensis are produced during natural infection in humans. Słowa kluczowe: receptory Toll-like, ekspresja, walidacja, real-time RT-PCR Introduction: Toll-like receptors (TLRs) are an important component of a innate immune system. Stimulation of TLRs, through action with helper T cells, could change Th1/Th2 balance and thus affect adaptive immune response. The aim of this work was to optimize the stimulation of the peripheral blood mononuclear cells (PBMC) and the validation of realtime RT-PCR method for determination of Toll-like gene expression in these cells. Methods: PBMCs from healthy donors were stimulated with measles viruses and ligands for TLR2 and TLR4. For examinations the real time RT-PCR (QuantiFast Assay, Qiagen) was used. Fold change of TLRs expression was normalized to GAPDH and estimated by 2 - Ct method. Validation of real-time RT-PCR method was performed for repeatability and efficiency. Results: The level of gene expression varies between individuals and was dose and time of incubation dependent. The efficiency of real-time RT-PCR was 90.4% ± 10.2 for GAPDH, 87.0% ± 8.2 for TLR2 and 44.5% ± 9.2 for TLR4. Repeatability, expressed as relative standard deviation (RSD) for Ct values was less than 0.70% for GAPDH, <3.2% for TLR2 and <2.84% for TLR4. Conclusions: Based on obtained results, the optimal conditions for stimulation were: 10 μg/ml/24h for LPS, 1μg/ml/6h for Pam3CSK4 and 1250 MeV infectious particles/24h. Słowa kluczowe: herpeswirusy, zakażenia potransplantacyjne, immunosupresja, real-time PCR Introduction: Infections caused with a variety of bacteria, fungi and viruses are still responsible for high level of mortality and morbidity in immunosupressed individuals. A case of fatal post-transplant reactivation with four herpesviruses in 49-year-old immunocompromised male with MDS-RAEB2, subjected to allogeneic haematopoietic stem cell transplantation was described. Methods: Full microbiological examination of was performed in different types of clinical samples (whole blood, stool). Sera specimens were tested for the presence of different viral DNA using the real-time PCR assays. Results and conclusions: DNA of HSV-1, VZV, HHV-6 and EBV in serum samples was detected using molecular biology techniques. Viral level of HSV-1 and VZV was constantly increasing despite routine applied oral acyclovir therapy. These findings underline the value of real-time PCR technique used in current therapeutic procedures and for monitoring of antiviral therapy with nucleoside analogs. We found that real-time PCR is a useful tool in detection and monitoring of disseminated herpesviral infection, especially for the detection of low-copy viraemia in clinical specimens. Słowa kluczowe: zakażenia grzybicze; oddział intensywnej terapii; ceftazydym Introduction: The objective of this study was to analyze the fungal infections in patients of an intensive care unit (ICU) in a long period (2002 2012) on the example of the Lodz Medical University Hospital No 1. This analysis was focused on the study of the effect of antimicrobial therapy on the level of these infections. Methods: A total of 291 strains of fungi were isolated from blood, tips of central intravenous catheters, lower respiratory tract, urine, wounds, pressure sores, and cerebrospinal fluid of 3177 patients. An automatic system Bactec 9050, Yeast ID Phoenix BD panels and E-tests (BioMerieux) were used for the fungi analysis. Results: The studies were mainly concentrated on the Candida infections, distinguishing cases caused by C. albicans and C. non-albicans pathogens. Changes in the number of these infections in consecutive years have been associated with epidemiological and therapeutic activities in the ICU. Particularly, relationships between the number of infections and the use of ceftazidime were discussed. A statistically significant positive correlation of the count of Candida infections and the ceftazidime consumption was found in the period to 2006. In the later years, the correlation was destroyed as a result of other important therapeutic factors (eg, immunosuppressive drugs). Conclusions: It has been found that the number of Candida infections in the ICU depends on the consumption of antimicrobial drugs. This conclusion is based on quantitative example of ceftazidime. Only close cooperation between the ICU and microbiologists is able to provide a reduction in nosocomial fungal infections. Keywords: fungal infections; intensive care unit; ceftazidime

Flora jelitowa stanowi bardzo ważny element w obronie organizmu przed zakażeniem patogenami przewodu pokarmowego, w tym pałeczkami Salmonella. Dlatego też drobnoustroje te wykształciły szereg mechanizmów mających na celu z jednej strony dostosowanie się do warunków panujących w przewodzie pokarmowym, z drugiej zaś strony zmieniają to środowisko, tak aby doszło do kolonizacji i internalizacji tych drobnoustrojów do komórek gospodarza. Those bacteria through a network of interactions and interdependencies, are integrated superorganism. The intestinal flora is a very important element in host s defense against infections of the gastrointestinal tract, caused by for example Salmonella. Therefore, this bacteria have evolved a number of mechanisms, which adapt pathogen to the conditions of the gastrointestinal tract, and on the other hand to the change this environment, for easier colonization and internalization into host cells. One of elements of mentioned above interactions are antimicrobial peptides produced by host s Paneths cells, which have antimicrobial feature. Salmonella mostly are resistant for those peptides, moreover they can stimulate AMPs production for increasing their abilities in competition for ecological niche. In case of Salmonella quorum sensing mechanism was also identified. It allows for recognition of other bacteria presence, which stimulate Salmonella for higher expression of SPI-1, SPI-4 genes. These genes encoded proteins are involved in many host-pathogens interaction, inter alia inflammatory induction. Using of antibiotics in case of Salmonella infections always cause dramatic changes in intestinal flora compositions, which facilitate Salmonella internalizations to host s cells and sometimes could even stimulate to this process. Antibiotic treatment could also cause increase of antimicrobial resistance. Also antibiotics influence on Salmonella carriage was confirmed. Moreover antibiotics could cause super-shedder phenotype, what was detected on streptomycin-treated mice with Salmonella carriage. Drobnoustroje te posiadają różne mechanizmy zjadliwości biorące udział w wywoływaniu zakażenia w organizmie człowieka. Do mechanizmów tych zalicza się przede wszystkim ruchliwość, zdolność do produkcji toksyn oraz inwazyjność. Prezentowany artykuł ma na celu przedstawienie aktualnej wiedzy na temat wybranych czynników chorobotwórczości pałeczek C. jejuni. Słowa kluczowe: Campylobacter jejuni, patogenność, czynniki zjadliwości Campylobacter jejuni is one of the most common causes of bacterial diarrhea worldwide. Various virulence factors of C. jejuni contribute to survival and participate in the induction of infection in the human body. The virulence mechanisms such as motility, toxin production or invasive properties allow the bacteria to survive in the human body.
