Biziela, Streszczenie Wprowadzenie. Reumatoidalne zapalenie stawów (RZS) jest najczêstsz¹ chorob¹ zapaln¹ stawów, w której dochodzi do postêpuj¹cej destrukcji struktur stawowych. Dlatego te poszukuje siê czu³ych i swoistych markerów, które by³yby przydatne w monitorowaniu przebiegu choroby. Cel pracy. Celem pracy by³a ocena stê enia wybranych autoprzeciwcia³ (anty-ccp, RF IgM i IgG) i hscrp u chorych na reumatoidalne zapalenie stawów (RZS) w ró nych stadiach zaawansowania choroby. Materia³ i metody. Badania przeprowadzono w grupie 75 chorych na RZS oraz w grupie kontrolnej 26 osób zdrowych. W surowicy krwi oznaczono, testami ELISA, stê enie przeciwcia³ przeciw cyklicznemu cytrulinowanemu peptydowi (anty-ccp), czynników reumatoidalnych w klasie IgM (RF IgM) i IgG (RF IgG) oraz metod¹ immunonefelometrii poziom bia³ka C-reaktywnego (hscrp). Wyniki. Stê enie anty-ccp, RF IgM i CRP by³o znamiennie wy sze u chorych z zaawansowanymi zmianami w porównaniu z wczesn¹ i umiarkowan¹ postaci¹ choroby. Wraz z zaawansowaniem choroby wzrasta³ odsetek wyników dodatnich oznaczanych parametrów. Stwierdzono wysok¹ zgodnoœæ wyników dodatnich (81%) anty-ccp z RF IgM u chorych z ciê k¹ i koñcow¹ postaci¹ RZS oraz wyników ujemnych (83%) u pacjentów z postaci¹ wczesn¹ i umiarkowan¹. U 14% chorych z zaawansowan¹ postaci¹ RZS, mimo ujemnych wyników RF IgM, przeciwcia³a anty-ccp tak e by³y dodatnie, podobnie jak bia³ko hs CRP. Wnioski. W praktyce klinicznej do oceny nasilenia stanu zapalenia i stopnia zaawansowania RZS, powinno byæ w³¹czone oznaczanie przeciwcia³ anty-ccp i hs CRP. Summary Introduction. Rheumatoid arthritis (RA) is the most common inflammatory joint disease, with progressive destruction of joint structures. Therefore, sensitive and specific markers, useful in monitoring the course of disease are extensively investigated. Aim of the study. The aim of the study was to evaluate the concentration of selected antibodies (anti-ccp, RF IgM and IgG) and hscrp in patients with rheumatoid arthritis (RA), in different degrees of the disease s progression. Material and methods. Seventy-five patients with rheumatoid arthritis and 26 healthy controls were included in the study. Anti-cyclic citrullinated peptide antibodies (anti-ccp), rheumatoid factor IgM (RF IgM) and IgG (RF IgG) were determined in the serum by ELISA and C- reactive protein (hscrp) using latex-enhanced immunonephelometric assays. Results. Concentration of anti-ccp, RF IgM and CRP was significantly higher in patients at an advanced stage, rather than in early and moderate stages of the disease. The percentage of positive results increased with the progression of the disease. We demonstrated a high conformability of positive results (81%) of anti-ccp with RF IgM in patients with severe and end-stage of RA and of negative results (83%) in patients with early and moderate stage. In 14% of RA patients in an advanced stage, in spite of negative RF IgM, anti-ccp antibodies and hs CRP, were also positive. Conclusions. In clinical practice, for an estimation of the degree of RA progression, measurement of anti-ccp antibodies and hscrp should be considered. Od lat trwaj¹ poszukiwania specyficznego markera serologicznego, który pozwoli³by na prognozowanie jej przebiegu. W 2002 r. Visser i wsp. [3] opracowali zestaw kryteriów prognostycznych, do których w³¹czono obecnoœæ przeciwcia³ przeciw cyklicznemu cytrulinowanemu peptydowi (anty-ccp), opisywanych w literaturze jako,,idealny marker reumatoidalnego zapalenia stawów ze wzglêdu na wysok¹ czu³oœæ i specyficznoœæ. Dowodzi siê zatem, e okreœlenie poziomu przeciwcia³ anty-ccp mo e byæ wartoœciowym wskaÿnikiem predykcyjnym aktywnej, nad erkowej postaci choroby, a w skojarzeniu z oznaczaniem RF IgM mo e przyczyniæ siê do selekcji chorych wymagaj¹cych wczesnego agresywnego leczenia farmakologicznego [4]. Celem pracy by³a ocena stê enia wybranych autoprzeciwcia³ (anty-ccp, RF IgM i IgG) i bia³ka C-reaktywnego, oznaczonego metod¹ o wysokiej czu³oœci, u chorych na reumatoidalne zapalenie stawów w ró nych stadiach zaawansowania procesu chorobowego. Rheumatoid arthritis (RA) is a systemic autoimmunological disease characterised by a chronic progressive inflammatory condition, initially appearing in the synovial membrane and in consequence leading to the destruction of the joints and impairment of their functions [1]. In the course of the disease often many extra-articular lesions and organic complications develop. Thus, in patients with RA, constant monitoring of the activity of the disease is crucial [2]. Research of a specific serologic marker, which would lead to the prognosing of its course, has been going on for years. In 2002, Visser et al. [3] evolved a set of prognostic criteria in which the presence of the anticyclic citrullinated peptide antibodies (anti-ccp), described in literature as a perfect marker of rheumatoid arthritis because of its high sensitivity and specificity, was included. It has been proved that the determination of the level of the anti-ccp antibodies may be a valuable predictive factor of the active erosive form of the disease, and in combination with the determination of RF IgM, may help to select patients requiring early aggressive pharmacological treatment [4]. The aim of the study was to evaluate the concentration of selected antibodies (anti-ccp, RF IgM and IgG) and hscrp in patients with rheumatoid arthritis (RA) in different degrees of the disease s progression. MATERIA I METODY Pacjenci Badaniem objêto grupê 75 pacjentów (68 kobiet i 7 mê czyzn) w wieku 25-80 lat z rozpoznanym reumatoidalnym zapaleniem stawów (RZS) oraz 26 osób zdrowych (17 kobiet i 9 mê czyzn) w wieku 28-60 lat. Rozpoznanie choroby zosta³o postawione przez lekarzy reumatologów na podstawie ustalonych kryteriów ARA/ACR obejmuj¹cych objawy kliniczne, zmiany radiologiczne i obecnoœæ/brak czynnika reumatoidalnego. Pacjentów z RZS podzielono wed³ug kryteriów Steinbrockera uwzglêdniaj¹cych 4 okresy zaawansowania choroby: I (wczesny) 11 chorych, II (umiarkowany) 35, MATERIAL AND METHODS Patients The study enrolled a group of 75 patients (68 women and 7 men) in the ages of 25-80 with diagnosed rheumatoid arthritis (RA), and 26 healthy individuals (17 women and 9 men) at the ages of 28-60. The diagnosis was made by rheumatologists based on the determined ARA/ACR criteria including clinical symptoms, radiological changes and the presence/lack of the rheumatoid factor. Patients with RA were randomised according to Steinbrocker s criteria taking into account the four stages of the progression: I (early) 11 patients, II (moderate) 35, III (severe) 21 and IV (end-stage) 8 patients. U wiêkszoœci pacjentów stosowano farmakoterapiê, tj. leki modyfikuj¹ce przebieg choroby, czyli LMPCh (metotreksat, sulfasalazynê, arechinê, sole z³ota), niesteroidowe leki przeciwzapalne, czyli NLPZ (Diclac, Dicloratio, Diclo Duo, Naklofen, Ketonal, Profenid, Piroxicam, Meloksam) oraz glikokortykosteroidy (Encorton, Metypred). 13 badanych by³o leczonych LMPCh przez 2 miesi¹ce do 25 lat, a 10 chorych NLPZ przez 8 miesiêcy do 14 lat. Leczenie skojarzone by³o stosowane u 11 pacjentów przez okres 3 miesiêcy do 20 lat (LMPCh i NLPZ), u 19 chorych (LMPCh i glikokortykosteroidy) i 9 badanych (LMPCh, glikokortykosteroidy i NLPZ ) przez 1 miesi¹c do 30 lat. Badania wykonano w surowicy krwi pobranej od pacjentów, w warunkach standardowych. Surowicê przechowywano w temperaturze 70 C do czasu wykonania oznaczeñ. W badanym materiale oznaczono testami ELISA stê enie przeciwcia³ przeciw cyklicznemu cytrulinowanemu peptydowi (anty-ccp) (EUROIMMUN), czynników reumatoidalnych w klasie M (RF IgM) i G (RF IgG) (IMTEC Immunodiagnostica GmbH) oraz poziom bia³ka C-reaktywnego (CRP) metod¹ immunonefelometrii o wysokiej czu³oœci przy u yciu zestawu odczynników CardioPhase hs CRP (Dade Behring). Przyjête wartoœci odciêcia wynosi³y dla anty-ccp 5 RU/ml, RF IgM 15 IU/ml, RF IgG 30 IU/ml i hs CRP 3 mg/l. Analizê statystyczn¹ otrzymanych wyników przeprowadzono za pomoc¹ testu U Mann a-whitney a dla prób niezale nych oraz korelacji liniowej Pearson a oraz Spearman a. Ze wzglêdu na rozk³ad wszystkich badanych zmiennych istotnie ró ni¹cy siê od rozk³adu normalnego wartoœci dla grupy badanej i kontrolnej podano w postaci mediany i kwartyli. the majority of patients pharmacotherapy was used, i.e. disease-modifying agents, i.e. DMA (methotrexate, sulfasalazine, arechine, aurum salts), non-steroidal antiinflammatory drugs, i.e. NSAIDs (Diclac, Dicloratio, Diclo Duo, Naklofen, Ketonal, Profenid, Piroxicam, Meloksam) and glycocorticosteroids (Encorton, Metypred). Thirteen subjects were treated with DMA for 2 months to 25 years, and 10 patients were treated with NSAIDs for 8 months to 14 years. The combined treatment was used in 11 patients for the period of 3 months to 20 years (DMA and NSAIDs), in 19 patients (DMA and glycocorticosteroids) and 9 subjects (DMA, glycocorticosteroids and NSAIDs) for 1 month to 30 years. Tests were carried out in the serum taken from patients, in standard conditions. The serum was stored at a temperature of +70 0 C until the moment of analysis. In the tested material, anti-cyclic citrullinated peptide antibodies (anti-ccp) (EUROIMMUN), rheumatoid factor IgM (RF IgM) and IgG (RF IgG) (IMTEC Immunodiagnostica GmbH) were determined in the serum by ELISA and C-reactive protein (hscrp) using latexenhanced immunonephelometric using a set of reagents CardioPhase hs CRP (Dade Behring) assay. The accepted cut-off values were for anti-ccp 5 RU/ml, RF IgM 15 IU/ml, RF IgG 30 IU/ml and hs CRP 3 mg/l. The statistical analysis of the obtained results was performed using the Mann-Whitney U test for the unrelated samples and Pearson and Spearman linear correlation. On the grounds of the distribution of all tested variables significantly different from the normal distribution values for the tested and control groups were given as the median and quartiles. WYNIKI Poziom przeciwcia³ anty-ccp w grupie chorych na RZS by³ istotnie wy szy w porównaniu z grup¹ kontroln¹ (tab.1). Pomimo, e mediana stê enia anty-ccp mieœci³a siê w zakresie wartoœci prawid³owych, to jednak u niektórych badanych (czwarty kwartyl) oznaczone stê enie przeciwcia³ by³o kilkunastokrotnie wy sze. U chorych na RZS mediana RF IgM by³a powy ej wartoœci odciêcia, z kolei mediana stê enia RF IgG mieœci³a siê w zakresie referencyjnym, jednak wartoœci obu czynników w najwy - szym kwartylu by³y wysokie. Stê enie bia³ka C-reaktywnego hs CRP oznaczone w grupie chorych na RZS by³o poni ej wartoœci granicznej, chocia mediana by³a wy sza ni w grupie kontrolnej. Poziomy CRP u chorych na RZS w najwy szym kwartylu ponad 3-krotnie przewy sza³y wartoœæ odciêcia. Mediany stê enia przeciwcia³ anty-ccp oraz RF IgM i CRP u pacjentów z III i IV RZS by³y wysokie, podczas RESULTS The level of anti-ccp antibodies in the group of patients with RA was significantly higher in comparison with the control group (table 1). Despite that, the median of the concentration of anti-ccp were in the range of the correct values, though in some patients (fourth quartile) the determined concentration of antibodies was about fifteen times higher. In patients with RA, the median of RF IgM was above the value of severance, and the median of RF IgG was in the reference range but the values of both factors in the highest quartile were high. The concentration of the C-reactive protein - hs CRP determined in the group of patients with RA was below the cut-off value; however, the median was higher than in the control group. The levels of CRP in patients with RA in the highest quartile were over 3 times higher than the value of severance. Stê enia przeciwcia³ anty-ccp i RF IgM w najwy szym kwartylu w RZS III i IV postacie ciê ka i koñcowa by³y wielokrotnie wy sze w porównaniu z postaci¹ wczesn¹ i umiarkowan¹. Wartoœci CRP by³y znamiennie wy sze u chorych z zaawansowanymi zmianami w porównaniu z wczesn¹, jak i umiarkowan¹ postaci¹ choroby. Stê enie tego wskaÿnika ró - ni³o siê, szczególnie, pomiêdzy I i IV RZS. W badanej grupie wraz z zaawansowaniem choroby (w stadium ciê kim i koñcowym) wzrasta³ odsetek wyników dodatnich oznaczanych parametrów a obni a³ siê odsetek wyników ujemnych. Najni szy odsetek wyników dodatnich (18%) we wczesnym stadium RZS (I ) stwierdzono dla anty-ccp i CRP, podczas gdy u chorych z postaci¹ koñcow¹ RZS (IV ) najwy szy by³ odsetek dodatnich wyników CRP (88%) (tab. 3). Dodatnie wyniki RF IgM i anty-ccp u pacjentów z postaci¹ ciê k¹ RZS obserwowano w 76 i 67% oraz z postaci¹ koñcow¹ w 71 i 63%. RA were high, while in patients with I and II, they did not exceed the reference values range (table 2). The concentrations of the anti-ccp antibodies and RF IgM in the highest quartile in RA III and IV - severe and endstage were multiple times higher in comparison with the early and moderate forms. The values of CRP were significantly higher in patients with advanced lesions in comparison with both early and moderate stages of the disease. The concentration of this factor differed significantly between I and IV RA. In the tested group together with the progresion of the disease (in severe or end-stage phase) the percentage of the positive results of the determined parameters increased, and the percentage of the negative results decreased. The lowest percentage of the positive results (18%) in the early stage of RA (I ) was determined for anti-ccp and CRP, while in patients with the end-stage form of RA (IV ) the highest was the percentage of the positive CRP results (88%), (table 3). Positive results of RF IgM and anti-ccp in patients with the severe form of RA were observed in 76 and 67%, and with the end-stage form in 71 and 63%. Tabela 1. Anty-CCP i hs CRP by³y dodatnie tak e u 14% chorych RF IgM (-) z postaci¹ ciê k¹ i koñcow¹ RZS. Z kolei we wczesnych i umiarkowanych stadiach choroby (I i II RZS) a u 83% pacjentów RF IgM(-) wystêpowa³y ujemne wartoœci przeciwcia³ anty-ccp, podczas gdy 43% mia³o stê enie CRP <3 mg/l. Table 4 shows the conformity of the positive/negative results of anti-ccp and CRP in seropositive (RF IgM+) and seronegative patients (RF IgM-) in relation to the level of severity. In 81% of the seropositive patients with III and IV RA, the positive values of the anti-ccp antibodies occurred. The increased >3mg/L values of CRP were characteristic for 54% of patients. Anti-CCP and hs CRP were also positive in 14% of patients RF IgM (-) with the severe and end-stage form of RA. In early and moderate stages of the disease (I and II RA), in 83% RF IgM (-) patients were found the negative values of the anti-ccp antibodies, while 43% had the concentration of CRP <3 mg/l. Ze wzglêdu na postêpuj¹cy charakter tej choroby istnieje koniecznoœæ agresywnego leczenia, które spowalnia destrukcjê stawów, ale jednoczeœnie czêsto doprowadza do wielu powik³añ narz¹dowych. Dlatego wci¹ poszukuje siê czu- ³ych i swoistych markerów, które by³yby przydatne w monitorowaniu przebiegu RZS [6]. Oznaczane w tej pracy przeciwcia³a przeciw cyklicznemu cytrulinowanemu peptydowi s¹ od pewnego czasu wykorzystywane w diagnostyce laboratoryjnej RZS. Wielu badaczy [7,8,9] t³umaczy obecnoœæ przeciwcia³ anty-ccp z wystêpowaniem lokalnej cytrulinacji ³añcuchów fibryny znajduj¹cych siê w ³uszczce reumatoidalnej. Bizzaro i wsp. [10] podkreœlaj¹, e wysokie stê enia tych przeciwcia³ s¹ wy³¹cznie zwi¹zane z wystêpowaniem RZS. DISCUSSION Rheumatoid arthritis (RA) is the most common inflammatory disease of joints [5]. Because of the progressive character of the disease, there is a necessity for the intensive treatment inhibiting the destruction of joints but, at the same time, causing many organic complications. Thus still sensitive and specific markers that would be useful in monitoring the course of RA have been researched [6]. The anti-cyclic citrullinated peptide antibodies determined in this study are used in the laboratory diagnostics of RA for some time. Many researchers [7, 8, 9] explain the presence of the anti-ccp antibodies by the presence of local citrullination of the fibrin chains present in the rheumatoid pannus. Bizzaro et al. [10] emphasize that high concentrations of these antibodies are connected exclusively with the development of RA.

Dane literaturowe wskazuj¹ na wystêpowanie anty-ccp we wczesnym stadium RZS u 30-45% badanych [11], a wg Vallbracht a i wsp. [12] nawet u 40-60% chorych. Czynnik reumatoidalny u chorych z RZS wystêpuje najczêœciej w klasie IgM, rzadziej w klasie IgG [13]. W niniejszej pracy obok RF IgM oznaczano tak e RF IgG metod¹ ELISA o wysokiej czu³oœci. RF IgM wykrywany jest w 20-30% przypadków wczesnego reumatoidalnego zapalenia stawów [14]. W badaniach w³asnych we wczesnym okresie choroby (I ) RF IgM(+) obecny by³ u 27% pacjentów, a RF IgG u 37% chorych. Wed³ug Steiner a i wsp. [11] czynnik reumatoidalny mo e wystêpowaæ we wczesnym stadium choroby nawet u 40-50% badanych. Zaobserwowano zale noœæ pomiêdzy obecnoœci¹ anty- CCP a stopniem nasilenia RZS. Odsetek wyników dodatnich w ciê kim i koñcowym stadium choroby by³ kilkakrotnie wy szy ni w fazie wczesnej (I ).Wskazuj¹ na to równie najnowsze dane z piœmiennictwa œwiatowego [12-17]. Wed³ug Lee i wsp. [18] i innych [19,20] u pacjentów z dodatnim wynikiem testu anty-ccp obserwuje siê nasilenie zapalenia stawów oraz wiêksze zmiany w obrazie radiologicznym, co prowadzi do destrukcji stawów. Wielu autorów podkreœla wartoœæ przeciwcia³ anty- CCP jako wskaÿnika prognostycznego aktywnoœci choroby [8,10,23]. W grupie chorych z ciê kim i koñcowym stadium (III i IV ) stê enia anty-ccp kilkakrotnie przekracza³y wartoœæ przyjêt¹ za prawid³ow¹ (47,25 i 37,18 RU/ml), a przeciwcia³a by³y obecne u 67 i 63% pacjentów. Wskazuje to, e pomimo leczenia, wraz z zaawansowaniem procesu chorobowego i zwi¹zanym z tym nasileniem zmian kostno-stawowych, u wiêkszej liczby chorych obecne s¹ przeciwcia³a anty-ccp, a ich miana s¹ zazwyczaj bardzo wysokie [21]. Wed³ug Saleem a i wsp. [22] kliniczna odpowiedÿ na leczenie nie ma zwi¹zku z obni eniem poziomu anty-ccp. Cruyssen i wsp. [23] wykazali, e ni szy poziom anty-ccp zwi¹zany jest z krótszym czasem trwania choroby, co potwierdzaj¹ wyniki w³asne otrzymane w grupie chorych z I i II RZS. Wskazuje siê równie na zwi¹zek RF z ciê koœci¹ zmian w RZS [7]. W badaniach w³asnych stwierdzono wzrost stê enia RF IgM wraz z zaawansowaniem choroby, pomimo stosowanego leczenia farmakologicznego, co potwierdza wczeœniejsze wyniki innych autorów [22]. RF IgM(+) by³ obecny u 71% pacjentów z postaci¹ koñcow¹ choroby. Odsetek dodatnich wyników anty-ccp i RF IgM by³ znacz¹co wiêkszy u pacjentów z III i IV RZS, u których In the group of patients with the early form of RA (I acc. to Steinbrocker s scale) lasting <2 years, the positive anti-ccp antibodies were found in 18% of the tested individuals. The literature data indicate the occurrence of the anti-ccp in the early stage of RA in 30-45% of the tested individuals [11], and acc. to Vallbracht et al. [12] even in 40-60% of patients. The rheumatoid factor in patients with RA is found the most commonly in the IgM class, less commonly in the IgG class [13]. In this study, besides RF IgM, RF IgG was also determined using the high sensitivity ELISA method. RF IgM is detected in 20-30% of cases of early rheumatoid arthritis [14]. In our own studies in the early stage of the disease (I ) RF IgM (+) were present in 27% of patients, and RF IgG in 37% of patients. According to Steiner et al. [11], the rheumatoid factor may occur in the early stage of the disease in even 40-50% of the tested individuals. The relation between the presence of anti-ccp and the level of the severity of RA was observed. The percentage of the positive results in the severe and endstage phases of the disease was several times higher than in the early phase (I ). The newest data from the international literature support the above [12-17]. According to Lee et al. [18] and the others [19, 20], in patients with the positive result of the anti-ccp test the intensification of the joint inflammation and larger lesions in the radiological picture which leads to the destruction of joints are observed. Many authors emphasize the value of the anti-ccp antibodies as the prognostic marker of the activity of the disease [8, 10, 23]. In the group of patients with severe and end-stage phase (III and IV ), the concentrations of anti-ccp were several times higher than the value accepted as normal (47.25 and 37.18 RU/ ml), and the antibodies were present in 67% and 63% of patients. It indicates that regardless of the treatment, together with the progression of the disease process and connected with it, the intensification of the osteoarticular lesions in the majority of patients the anti-ccp antibodies are present, and their titres are usually very high [21]. According to Saleem et al. [22], the clinical response to the treatment does not have any relation with the reduction of the level of anti-ccp. Cruyssen et al. [23], proved that the lower level of anti-ccp is connected with the shorter duration of the disease, which is proved by our own results obtained in a group of patients with I and II RA. The relation between the RF and the severity of lesions in RA is also indicated [7]. In our own studies, the increase of the concentration of RF IgM, together with the progression of the disease regardless of the used pharmacological treatment, which proves the earlier results by other authors has been observed [22]. RF IgM(+) was present in 71% of patients with the end-stage phase of the disease. The percentage of the positive results of anti-ccp and RF IgM was significantly higher in patients with III

Wysoka zgodnoœæ wystêpowania dodatnich anty-ccp u chorych RF IgM(+) i ujemnych anty-ccp u pacjentów RF IgM(-) w zale noœci od stopnia ciê koœci choroby wskazuje na zwi¹zek obu rodzajów przeciwcia³ z klinicznymi objawami RZS. Obecnoœæ dodatnich wyników anty-ccp u osób z zaawansowanymi postaciami choroby, ale RF IgM (-) wskazuje na koniecznoœæ poszerzenia diagnostyki serologicznej w RZS. W przewlek³ych chorobach zapalnych, w tym RZS, dochodzi do wzrostu stê enia bia³ek ostrej fazy, takich jak: CRP [24,25]. Wed³ug niektórych autorów [22,26] u pacjentów z RZS mo e wystêpowaæ stan zapalny o niewielkim nasileniu, dlatego te s³uszne wydaje siê oznaczanie bia³ka C-reaktywnego metod¹ o wysokiej czu³oœci. Badania Dessein a i wsp. [26] i innych [22,27] wskazuj¹ na zwi¹zek hs CRP z aktywnoœci¹ choroby w RZS, co tak e potwierdzaj¹ otrzymane wyniki. Istotnie ni sze wartoœci CRP zaobserwowano u chorych z I i II RZS w porównaniu do pacjentów, u których choroba by³a zaawansowana (III i IV ), co sugeruje ostry jej przebieg mimo stosowanego leczenia. Autorzy niniejszej pracy nie mieli mo liwoœci oceny aktywnoœci choroby na podstawie wskaÿnika DAS, który bierze pod uwagê m. in. liczbê bolesnych i obrzêkniêtych stawów. Jednak e zaobserwowany najwy szy odsetek (88%) wyników CRP >3mg/L wœród chorych z postaci¹ koñcow¹ RZS, a tym samym najni szy odsetek (12%) niskich stê eñ tego wskaÿnika i brak istotnego nak³adania siê wartoœci CRP, najwy szych w I i najni szych w IV RZS, wskazuj¹ na mo liwoœæ wykorzystania oznaczania CRP metod¹ o wysokiej czu³oœci do oceny nasilenia procesu zapalnego, który pozostaje w zwi¹zku z kryteriami prognostycznymi i nastêpstwami choroby, co równie proponuj¹ inni autorzy [13,28,29]. WNIOSKI Przewa aj¹ca wiêkszoœæ chorych w stadium ciê kim i koñcowym RZS cechowa³a siê znacznie podwy szonym stê eniem anty-ccp, RF IgM oraz CRP w porównaniu do postaci wczesnej choroby. Stwierdzono wysok¹ zgodnoœæ wystêpowania dodatnich lub ujemnych wyników RF IgM i anty-ccp w zale noœci od stopnia ciê koœci choroby, co wskazuje na zwi¹zek obu rodzajów przeciwcia³ z klinicznymi objawami RZS. Sugeruje siê, e w praktyce klinicznej do oceny nasilenia stanu zapalenia i stopnia zaawansowania RZS, powinno byæ w³¹czone oznaczanie przeciwcia³ anty-ccp i hscrp. and IV RA, which have evident symptoms of joint damage. The high conformity of occurrence of the positive anti-ccp in patients RF IgM(+) and negative anti-ccp in patients RF IgM(-), depending on the level of severity of the disease, indicates the relation of both types of antibodies with the clinical symptoms of RA. The presence of the positive results of anti-ccp in individuals with advanced forms of the disease but RF IgM (-), indicates the necessity of the broadening of the serological diagnostics in RA. In chronic inflammatory diseases, including RA, the growth of the acute phase proteins occurs, i.e.: CRP [24, 25]. According to some authors [22, 26], in patients with RA the inflammatory condition of insignificant intensity may occur. Thus, it seems to be right to determine the high sensitivity C-reactive protein. The studies by Dessein et al. [26] and the others [22,27] indicate the relation between the hs CRP and the activity of the disease in RA, which is proven by the obtained results. Significantly lower values of CRP have been observed in patients with I and II RA in comparison to patients with advanced disease (III and IV ), which suggests its acute course, regardless of the used treatment. The authors of this paper did not have the ability to asses the activity of the disease based on the DAS factor, which takes into account the number of painful and swollen joints. However, the highest observed percentage (88%) of the results of CRP >3mg/L in patients with the endstage phase of RA and at the same time the lowest percentage (12%) of the low concentrations of this factor and the lack of significant overlapping of the values of CRP, the highest in I and the lowest in IV RA, indicate the possibility of usage of the determination of CRP using the high sensitivity method for assessment of the severity of the inflammatory process, which is related to the prognostic criteria and the consequences of the disease, as proposed by other authors [13,28,29]. CONCLUSIONS The majority of patients with severe and end-stage of RA had a significantly increased concentration of anti- CCP, RF IgM and CRP in comparison with the early phase of the disease. The high conformity of the occurrence of the positive or negative results of RF IgM and anti-ccp depending on the level of severity of the disease was observed. It indicates the relation of both types of antibodies with the clinical symptoms of RA. It is suggested that in the clinical practice for the assessment of the severity of the inflammatory condition and the progression of RA the determination of the anti-ccp and hscrp should be included.
