mgr Małgorzata Zgoda Ocena umiejętności edukacyjnych użytkowników systemu implantu ślimakowego Streszczenie Wstęp Wykształcenie i następnie zatrudnienie na rynku pracy odgrywają kluczową rolę w kształtowaniu dobrostanu psychologicznego i społeczno-ekonomicznego ludzi. Dobre wykształcenie przekłada się na szerszy wachlarz możliwości podczas dokonywania wyborów życiowych, daje szanse rozwijania talentów i zaspokajania aspiracji, gdyż wiedza stanowi przyczynę uruchomienia i absorpcji czynników wzrostu. Dane statystyczne oraz badania pokazują znacznie niższe wskaźniki wykształcenia oraz aktywności zawodowej osób niepełnosprawnych. Niepełnosprawnością, która w szczególny sposób wpływa na możliwość zdobycia wykształcenia i podjęcie zatrudnienia, jest niepełnoprawność słuchowa. Dzięki zastosowaniu implantu ślimakowego możliwa jest znacznie skuteczniejsza niż kiedykolwiek wcześniej kompensacja wady słuchu i przeciwdziałanie negatywnym konsekwencjom niepełnosprawności słuchowej. Pozytywne wyniki rehabilitacji słuchu i mowy po wszczepieniu implantu ślimakowego pozwalają oczekiwać poziomu osiągnięć szkolnych uczniów porównywalnego do tego, jaki osiągają dzieci ze słuchem prawidłowym. Dotychczas jedynie nieliczne doniesienia w sposób zobiektywizowany dokumentowały zagadnienie realizacji wymagań uczniowskich i osiąganych wyników szkolnych przez dzieci korzystające z implantów ślimakowych. Brak jest także wiedzy o czynnikach mających wpływ na poziom umiejętności edukacyjnych osiągany przez uczniów korzystających z implantu ślimakowego. Cel pracy Celem pracy było zbadanie osiągnięć szkolnych uczniów z niedosłuchem prelingwalnym, zaopatrzonych w implanty ślimakowe, kończących początkowy etap edukacji.
Dla zrealizowania celu głównego wyznaczono dwa cele szczegółowe: Cel szczegółowy 1 Określenie parametrów rozkładu wyników sprawdzianu (wyniku ogólnego oraz wyników w zakresie umiejętności czytania, pisania, rozumowania, korzystania z informacji oraz wykorzystania wiedzy w praktyce) dla testu w wersji standardowej i w wersji dostosowanej. Cel szczegółowy 2 Dokonanie porównania wyników grupy badanej w poszczególnych latach z wynikami populacji dzieci w Polsce piszących sprawdzian na zakończenie nauki w szkole podstawowej. Ze względu na to, że badania prowadzone w grupie dzieci ze słuchem prawidłowym wykazały istnienie wpływu czynników indywidualnych, środowiskowych oraz pedagogicznych na osiągnięcia szkolne, w niniejszej pracy postawiono hipotezę, że także w przypadku dzieci korzystających z implantu ślimakowego istnieją określone czynniki, które mogą być związane z poziomem wyników nauczania na etapie edukacji początkowej. Aby przetestować prawdziwość postawionej hipotezy, sformułowano trzeci cel szczegółowy. Cel szczegółowy 3 Przeprowadzenie analizy porównawczej osiągnięć szkolnych między wyłonionymi grupami dzieci z implantami, różniących się pod względem określonych na podstawie dotychczasowej wiedzy czynników indywidualnych, środowiskowych i pedagogicznych. Wyodrębnione czynniki grupujące, związane ze statystycznie istotnymi różnicami w wynikach edukacyjnych, będą mogły być traktowane jako predyktory osiągnięć szkolnych dzieci korzystających z systemów implantów ślimakowych. Materiał i metoda Grupę badaną stanowiło 160 dzieci z niedosłuchem prelingwalnym, którym wszczepiono implanty ślimakowe w Instytucie Fizjologii i Patologii Słuchu. W celu wykonania analiz statystycznych uzyskano dostęp do danych surowych wyników sprawdzianu na zakończenie nauki w szkole podstawowej wszystkich uczniów polskich piszących sprawdzian w latach 2010-2014 w wersji standardowej oraz dostosowanej dla słabosłyszących i niesłyszących. W badaniach wykorzystano także ankietę pocztową własnej konstrukcji, wyniki skali sprawności słuchowej Categories of Auditory Performance oraz dane medyczne pacjenta. Wyniki
Osiągnięcia szkolne uczniów z niedosłuchem prelingwalnym, zaopatrzonych w implanty ślimakowe, kończących początkowy etap edukacji: - w grupie piszącej sprawdzian na formularzu standardowym nie różnią się statystycznie istotnie pod względem osiągnięć szkolnych w porównaniu z populacją dzieci bez dysfunkcji słuchowej, - w grupie piszącej sprawdzian na formularzu dostosowanym nie różnią się statystycznie istotnie pod względem osiągnięć szkolnych w porównaniu z populacją dzieci z niedosłuchem umiarkowanego stopnia. Czynnikami wpływającymi w sposób znaczący na wyniki szkolne (wpływ na wynik ogólny sprawdzianu lub co najmniej dwie kategorie sprawdzianu) są: 1) W grupie czynników indywidualnych: wiek w momencie operacji wszczepienia implantu, długość czasu korzystania z urządzenia, poziom sprawności słuchowej, znajomość języka migowego, przyczyna wystąpienia wady słuchu, występowanie padaczki, występowanie wady wzroku. 2) Wśród czynników pedagogicznych: typ szkoły podstawowej, do jakiej uczęszczało dziecko, powtarzanie nauki, posiadanie przez dziecko orzeczenia o potrzebie kształcenia specjalnego lub indywidualnego nauczania. 3) Związane z czynniki środowiskowymi: wykształcenie zarówno matki, jak i ojca ucznia z implantem oraz miejsce zamieszkania ucznia. Wnioski Przeprowadzone badania będące celem niniejszej rozprawy doktorskiej uzupełniły informacje na temat osiągnięć edukacyjnych dzieci z niedosłuchem prelingwalnym, korzystających z implantów ślimakowych. Uzyskane w pracy wyniki pozwalają na sformułowanie następujących wniosków: 1) Zastosowanie implantu ślimakowego u dzieci z głuchotą i głębokim niedosłuchem może całkowicie wyeliminować lub znacznie ograniczyć negatywne skutki niepełnosprawności słuchowej w zakresie funkcjonowania szkolnego. 2) Wyodrębnione czynniki indywidualne, pedagogiczne i środowiskowe, które wpływają na osiągnięcia edukacyjne, mogą być uznane za determinanty indywidualnego potencjału edukacyjnego dzieci z niedosłuchem prelingwalnym, korzystających z implantów ślimakowych. 3) Wykazany pozytywny wpływ wszczepienia implantu do drugiego roku życia oraz dłuższej rehabilitacji słuchowej na wyniki szkolne uzasadnia konieczność wczesnej
interwencji medycznej z wykorzystaniem implantu ślimakowego u dzieci z prelingwalnym głębokim niedosłuchem lub całkowitą głuchotą. 4) Zaobserwowane lepsze wyniki edukacyjne dzieci o wyższym poziomie sprawności słuchowej wskazują na to, że kompetencje słuchowe i komunikacyjne osiągane za pomocą implantu ślimakowego przekładają się na sukces edukacyjny. Działania wspierające rozwój sprawności słuchowej i komunikacyjnej zwiększają szanse na lepsze osiągnięcia szkolne. 5) Wyniki osiągane przez dzieci z implantami ślimakowymi uczące się w placówkach ogólnodostępnych potwierdzają możliwość ich efektywnej edukacji w tego typu szkolnictwie. 6) Zidentyfikowane w niniejszej pracy determinanty indywidualnego potencjału edukacyjnego uczniów korzystających z implantów ślimakowych powinny być uwzględnione przy planowaniu wielodyscyplinarnych działań pomocowych i profilaktycznych w obszarach: zdrowotnym, pedagogicznym i środowiskowym, ukierunkowanych na wyrównanie szans rozwojowych dzieci z niepełnosprawnością słuchową.
The assessment of academic achievements of cochlear implant users Summary Introduction Education and subsequently employment are fundamental for individuals psychological and socio-economic wellbeing. Good education translates into the wider range of options when making life decisions, creates opportunities for developing talents and fulfilling one s ambitions, since knowledge facilitates mobilization and absorption of growth factors. Statistical data and studies show much lower rates of education and employment among the disabled. A hearing impairment is the kind of disability with a particular impact on education and employment opportunities. With application of cochlear implants it is possible to compensate for a hearing loss much more effectively than before thus better counteracting the negative consequences of hearing impairment. Positive effects of rehabilitation of hearing and speech after cochlear implantation allow expecting that these students will achieve similar school education results as children with normal hearing. To date only a few study reports had included an objective record of the issues related to complying with the school education requirements and school scores achieved by children using cochlear implants. Knowledge about factors influencing the educational achievements of implanted students is also scarce. Aim The aim of this study was the analysis of school attainments of children with the prelingual hearing loss using cochlear implants who were finishing the primary level of school education. In order to achieve the primary aim, two specific goals have been established: Specific goal 1 To define the parameters of distribution of the scores of a school test (overall score and specific scores related to the particular skills of reading, writing, reasoning, using the information and practical application of knowledge) in a standard and adjusted version. Specific goal 2
To compare the results obtained by the study group in subsequent years with the results obtained by a population of children writing the test at the end of primary school education in Poland. In relation to the finding that school achievements in a group of children with normal hearing are influenced by factors relating to the individual, environmental and educational conditions, this paper has put forward a hypothesis that also with regards to children using cochlear implants there exist specific factors which may be related to the level of the primary education scores. In order to verify this hypothesis, a third specific goal has been established. Specific goal 3 Comparative analysis of school achievements in the specific groups of implanted children differing in terms of the individual, environmental and educational factors defined based on the hitherto knowledge. Separated grouping factors related to the statistically relevant differences in educational scores should be considered as the predictors of school achievements of children using cochlear implants. Material and method The study group comprised 160 children with the prelingual deafness provided with cochlear implants in the Institute of Physiology and Pathology of Hearing. In order to perform the statistical analysis, author has obtained the raw data of the results of test at the end of primary school for all Polish students writing this test in years 2010-2014 in the standard version and in the eversion adjusted for hard-of-hearing and deaf students. In the analysis were included also the results of the postal survey of own design, results of the scale Categories of Auditory Performance and patients medical data. Results Educational achievements of students with the prelingual deafness using cochlear implants who were finishing the primary level of education: - in the group writing the test in a standard form are not in a statistically significant way different compared to the population of children without impairments, - in the group writing the test in an adjusted form are no different in a statistically significant way compared to the population of children with a moderate level hearing impairment.
Factors exerting a significant influence on the school results (influencing the overall score and at least two sub-categories of the test) are: 1) In the group of individual factors: age at implantation, duration of cochlear implant experience, level of hearing proficiency, knowledge of sign language, aetiology of hearing impairment, epilepsy, visual impairment. 2) Among the educational factors: type of the primary school, grade repetition, having the decision about the need of special or individual education. 3) Related to the environmental factors: education level, both of a mother and of a father of an implanted student and place of residence of a student. Conclusions Studies conducted in order to achieve the goals specified for this doctoral dissertation have complemented the available information about the educational achievements of children with the prelingual deafness using cochlear implants. Results obtained in this study allow formulating the following conclusions: 1) Providing a child with deafness or profound hearing loss with a cochlear implant may result in complete elimination or a substantial reduction of hearing impairment with regard to its school functioning. 2) Identified in this study individual, educational and environmental factors influencing the educational performance may be considered as the determinants of an individual educational potential of children with a prelingual hearing loss using cochlear implants. 3) Demonstrated positive results of cochlear implantation before a child is 2 years old and beneficial effects of longer rehabilitation after cochlear implantation reflected by school achievements justify the need of an early medical intervention with a cochlear implant in children with the prelingual profound or total hearing loss. 4) Observed better educational results of children with higher levels of hearing proficiency suggest that auditory and communication competences achieved with the aid of cochlear implant translate into an academic success. Therefore any activities promoting and supporting the development of hearing and communication proficiency create a chance for better school achievements. 5) Good results achieved by implanted children attending mainstream schools confirm that it is possible to successfully educate them in this type of school system.
6) The determinants of individual educational potential of students using cochlear implants identified in this study should be taken into consideration when planning the interdisciplinary help and multidisciplinary assistance and prevention activities in the spheres of health, education and social environment directed at equalization of chances of children with hearing impairment.
