Streszczenie Wstęp Oczopląs to mimowolne, rytmiczne ruchy gałki ocznej. Występowanie oczopląsu patologicznego szacuje się na 0,2% w populacji. W leczeniu wykorzystuje się metody chirurgiczne i zachowawcze. Dotychczas opublikowano niewiele prac poświęconych jakości życia osób z oczopląsem. Cel Pierwszym celem pracy było wykorzystanie autorskiego kwestionariusza do zbadania jakości życia dorosłych osób z oczopląsem o dobrym ogólnym stanie zdrowia. Drugim celem pracy była ocena występowania uprzedzeń wobec osób z oczopląsem. Materiały i metody Opracowanie kwestionariusza oparto na przeglądzie literatury i rozmowach z osobami z oczopląsem (7 osób) oraz specjalistami (2 okulistów, optometrysta i tyflopedagog pracujący z osobami słabowidzącymi). Sporządzono wstępną listę pytań, która po selekcji pozwoliła na opracowanie kwestionariusza składającego się z 26 pytań dotyczących funkcji wzrokowych oraz 26 pytań dotyczących funkcji psychospołecznych. Ocena kwestionariusza składała się z dwóch faz. W badaniu pilotażowym 34 dorosłe, zdrowe osoby z oczopląsem wypełniły kwestionariusz. Za pomocą analizy Rascha oceniono rzetelność, zastosowane kategorie odpowiedzi i dopasowanie danych do modelu. W ten sposób powstała skrócona wersja ankiety ze zredukowaną z 5 do 4 liczbą kategorii odpowiedzi. W drugiej fazie badania kwestionariusz wypełniło 50 dorosłych, zdrowych osób z oczopląsem. Oceniono jednowymiarowość kwestionariusza i dopasowanie zadań testowych do respondentów. Na podstawie oceny 10 ekspertów przeprowadzono badanie trafności mające wykazać, czy pytania zawarte w kwestionariuszu są właściwą konceptualizacją wpływu oczopląsu na jakość życia. W badaniu poświęconym występowaniu uprzedzeń wobec osób z oczopląsem przygotowano dwa krótkie filmy prezentujące osobę ubiegającą się o prace podczas rozmowy kwalifikacyjnej. Jedyna różnica między filmami polegała na tym, że na jednym z zarejestrowanych obrazów wideo u kandydatki wywołano oczopląs optokinetyczny. Następnie zaprezentowano 163 respondentom losowo wybrany film i poproszono o ocenę kandydatki pod względem 11 cech. Wyniki
W efekcie dwóch faz badań powstał kwestionariusz składający się z 24 pytań podzielonych na trzy podskalne. Skala ogólnych funkcji wzrokowych składa się 9 pytań, osiągnęła współczynnik rzetelności 0,8824 i indeks rozłączności respondentów 2,7389. Skala funkcji wzrokowych do bliży składa się z 5 pytań, posiada rzetelność 0,8642 i indeks rozłączności respondentów 2,5225. Skala funkcji psychospołecznych osiągnęła rzetelność 0,8458 i indeks rozłączności respondentów 2,342. Wszystkie skale posiadały założony wskaźnik trafności, bardzo dobre dopasowanie zadań testowych do respondentów oraz danych do modelu. W badaniu dotyczącym występowania uprzedzeń wobec osób z oczopląsem (po korekcie Bonnferoniego) uzyskano różnice istotne statystycznie w odniesieniu do kompetencji, umiejętności komunikacyjnych, niezawodności, inteligencji i umiejętności organizacyjnych. Dyskusja Osoby z oczopląsem z powodu obniżonych funkcji wzrokowych mogą mieć problem w wykonaniem codziennych czynności, co ogranicza ich samodzielność. Statyczna ostrość wzroku badana na optotypach jest niewystarczającym kryterium do podejmowania decyzji medycznych u pacjentów z oczopląsem. Z badań, w których zastosowano nowy, skonstruowany przeze mnie kwestionariusz wynika, że oczopląs ma negatywny wpływ na umiejętności interpersonalne. Badania wskazują na występowanie uprzedzeń wobec osób z oczopląsem, co może utrudniać im osiągnięcie wyznaczonych celów pomimo posiadanych kompetencji. Nowy skonstruowany kwestionariusz być może stać się użytecznym narzędziem wykorzystywanym zarówno podczas prowadzenia badań, jak i w praktyce klinicznej. Wnioski: 1. Dwie najważniejsze dziedziny jakości życia, na które wpływa oczopląs to funkcje wzrokowe i funkcje psychospołeczne. 2. Dziedziną funkcji psychospołecznych, na którą oczopląs ma największy negatywny wpływ, są umiejętności interpersonalne. 3. Osoby z oczopląsem z powodu niższego wskaźnika symetrii i przeciętności twarzy oraz nieprawidłowo utrzymywanego kontaktu wzrokowego są oceniane gorzej pod względem cech osobowości.
Summary Introduction Nystagmus is a condition of involuntary, rhythmic movements of the eyeball. The incidence of pathological nystagmus is estimated at 0.2% of the population. Surgical and conservative methods are used in the treatment. So far, few papers have been published devoted to the quality of life of patients with nystagmus. Objective The first aim of the study was to examine the health-related quality of life in healthy adults living with nystagmus using the newly developed questionnaire. The second aim of the study was to evaluate the prejudices against people with nystagmus. Material and methods Based on a review of literature and conversations with patients with nystagmus (7 people) and specialists (two ophthalmologists, an optometrist and a specialist from a facility for visually impaired) an initial list of questions has been prepared. After the selection, a questionnaire was created consisting of 26 questions about visual functions and 26 questions about psychosocial functions. In the pilot study 34 healthy adults with nystagmus completed the questionnaire. The reliability, response categories and data matching to the model were evaluated using the Rasch analysis. A shortened version of the questionnaire was developed with a number of response categories reduced from 5 to 4. At stage two of the evaluation process, the questionnaire was completed by 50 healthy adults living with nystagmus. Reliability, unidimensionality, fit statistic as well as targeting of items to the respondents were assessed. Based on evaluation conducted by 10 experts, the questionnaire s validity was assessed to check whether the questions included in the questionnaire presented the right concept of the impact of nystagmus on the HRQoL. In the study devoted to the prejudices against patients with nystagmus two short films were prepared which presented the person applying for work during an interview. The only difference between the two films was optokinetic nystagmus induced in one of the candidates recorded in video images. Then, 163 participants were shown with a randomly selected film and asked to evaluate the candidate in terms of 11 features.
Results At stage two of the study, a questionnaire was developed which consisted of 24 questions, divided into three subscales. The general visual functions scale consisted of 9 questions and achieved the reliability ratio of 0.8824 and person separation index of 2.7389. The near-vision visual function scale consisted of 5 questions and achieved the reliability ratio of 0.8642 and person separation index of 2.5225. Whereas the psychosocial functions scale consisted of 9 questions and achieved the reliability ratio of 0.8658 and person separation index of 2.342. All scales reached the expected validity ratio, showed a good targeting and fit statistic. In the study on the prejudices against patients with nystagmus (after The Bonnferoni's correction) statistically significant differences in the candidate's assessment in terms of competence, communication skills, reliability, intelligence and organizational skills. Discussion Patients with nystagmus, because of the reduced visual functions, may have problem with performing everyday actions, what limits their self-reliance. The assessment of static visual acuity tested on optotypes is definitely an insufficient criterion for making medical decisions in patients with nystagmus. The research performed with newly developed questionnaire prove that the nystagmus has got the negative influence on the interpersonal abilities. The research indicate the existence of prejudices against patients with nystagmus which is why they may encounter problems with achieving their goals despite having suitable predispositions. The newly developed questionnarie can be a very useful tool used in experimental trials and clinical practice Conclusions 1. The two main areas of the quality of life affected by nystagmus are visual functions and psychosocial functions. 2. Studies show that the area of psychosocial functions most negatively affected by nystagmus is interpersonal skills.
3. People with nystagmus are often perceived less favourably regarding their personality traits because of the lower face symmetry and face averageness and because of incorrectly sustained eye contact.
