Mnich Summary Aim of the study: To evaluate microbiological effectiveness of full-mouth disinfection protocol with the use of Elugel and Listerine mouthrinse in the initial phase of chronic periodontitis therapy. Material and methods: The study material consisted of 20 patients with diagnosed chronic periodontitis. Every person had scaling with root planing performed during the initial visit augmented with full-mouth disinfection. The study parameters included PI, API, SBI, BOP, PPD and CAL. Additionally, 10 patients from each group had microbiological tests to detect 9 periodontopathogenes using real-time PCR. Clinical and microbiological examination was made three times. Results: There was a statistically significant decrease in all the examined parameters. Conclusions: The application of the FMD protocol effectively improved clinical and microbiological parameters in the examined patients. Streszczenie Cel pracy: kliniczna i mikrobiologiczna ocena skuteczności odkażania jamy ustnej z użyciem żelu chlorheksydynowego Elugel i płynu Listerine we wstępnej fazie leczenia przewlekłego zapalenia przyzębia. Materiał i metody: materiał stanowiło 20 pacjentów z rozpoznanym przewlekłym zapaleniem przyzębia. U wszystkich osób na pierwszej wizycie wykonywano skaling wraz z wygładzeniem powierzchni zębów i korzeni oraz odkażaniem jamy ustnej. Oznaczono wskaźniki: PI, API, SBI, BOP oraz PPD i CAL. Dodatkowo u 10 osób z badanej grupy wykonano oznaczenie 9 periopatogenów metodą real-time PCR. Badanie kliniczne jak i mikrobiologiczne wykonano trzykrotnie. Wyniki: uzyskano statystycznie istotne obniżenie wszystkich wartości badanych parametrów. Wnioski: zastosowanie protokołu odkażania jamy ustnej spowodowało poprawę parametrów klinicznych oraz mikrobiologicznych w badanej grupie pacjentów. J Stoma Introduction Today s approach to the aetiology of periodontal inflammations includes three groups of factors: individual susceptibility, absence of the so-called beneficial bacterial species and the presence of pathogenic species of bacteria in the biofilm. 1,2 Since it is impossible to modify host s susceptibility to periodontal disease, periodontal treatment is based on periodonopathogen reduction and the restoration of conditions favourable to physiological flora within the pockets. There are approximately 400 or more species of bacteria that can be isolated in the subgingival plaque. 3-7 Cultures, DNA and immunological tests have demonstrated that some species of micro-organisms tend to co-exist. Basing on this observation, Socransky et al. 6 managed to identify five complexes of bacteria: the red complex: Porphyromonas gingivalis, Treponema denticola and Tannarella forsythia, the orange complex: Fusobacterium nucleatum, Prevotella intermedia, Prevotella nigrescens, Parvimonas micra (s, Peptostreptococcus micros), Campylobacter rectus, Campylobacter showa, Campylobacter gracilis, Eubacterium nodatum, Streptococcus constellatus, the green complex: Capnocytophaga, Campylobacter concisus, Eikenella corrodens, Aggregatibacter actinomycetemcomitans serotype a (s. Actinobacillus actinomycetemcomitans serotype a) treptococcus, the yellow complex: S. mitis, S. sanguis, S. oralis, the purple complex: Actinomyces odontolyticus and Veillonella parvula, Wstęp Współcześnie w podejściu do etiologii zapaleń przyzębia rozważa się trzy grupy czynników: podatność osobnicza, brak tak zwanych pożytecznych gatunków bakterii oraz obecność patogennych gatunków bakterii w biofilmie. 1,2 W związku z brakiem możliwości modyfikowania podatności gospodarza na choroby przyzębia, leczenie periodontologiczne jest oparte na redukcji periodontopatogenów i przywróceniu w kieszonkach warunków odpowiednich dla fizjologicznej flory bakteryjnej. Z poddziąsłowej płytki nazębnej można wyizolować 400 lub więcej gatunków bakterii. 3-7 Posiewy, badania DNA i immunologiczne dowiodły, że niektóre gatunki drobnoustrojów częściej występują wspólnie. 2011, 64, 5-6 Chronic periodontitis However, Aggregatibacter actinomycetemcomitans serotype b (s. Actinobacillus actinomycetemcomitans serotype b), Selenomonas noxia and Actinomyces naeslundi never occur in tandem, nor are they present in any group. Basing on the research, the following characteristic features have been proven: infection with the red complex bacteria is always preceded by the colonization with orange complex bacteria. Yellow, green and purple complex bacteria with the addition of Actinomyces are at the forefront of biofilm colonization, usually preceding the colonization with Gram negative bacteria of the orange and red complexes. 6-8 The infection with the red complex bacteria is markedly related to the pocket depth and bleeding on probing. The deeper the periodontal pockets, and the more intense bleeding, the higher the concentration of the red complex bacteria. Scaling and root planing (SRP) has a significant impact on the red complex bacteria, and hardly any the others. 6 In order to enhance the efficacy of the basic, non-surgical treatment methods, a new concept has been proposed based on full-mouth disinfection. 9,10 If the procedure is performed within 24 hours, it prevents the cross-infection from the untreated pockets to the already cleansed sites. Besides, the antibacterial effect is reinforced by the action of antiseptics. Even though the effect of irrigation on the subgingival flora is negligible, it enhances the therapeutic effect by eliminating pathogens from ecological niches other than pockets. 2,10,11 The concept of full-mouth disinfection is based on evidence concerning the infective aetiology of periodontal diseases. Such a treatment is also beneficial to the patient since it eliminates the need of several visits. Quirynen et al. 12 suggested that such a therapy can be more effective in patients who have low level of co-operation. The disadvantage Z kolei Aggregatibacter actinomycetemcomitans serotyp b (s. Bakterie kompleksu żółtego, zielonego i purpurowego oraz Actinomyces wcześnie kolonizują biofilm, zwykle poprzedzając kolonizację Gram ujemnymi bakteriami kompleksu pomarańczowego i czerwonego. 6-8 Zakażenie bakteriami kompleksu czerwonego wykazuje silny związek z głębokością kieszonek i krwawieniem podczas zgłębnikowania. Im głębsze kieszonki przyzębne i większe krwawienie, tym więcej bakterii kompleksu czerwonego. Oczyszczenie i wygładzenie powierzchni koron i korzeni zębów (scaling and root planing SRP) ma duży wpływ na bakterie kompleksu czerwonego i prawie żadnego wpływu na pozostałe. 6 Aby zwiększyć skuteczność podstawowego, niechirurgicznego leczenia, stosuje się nową koncepcję polegającą na pełnym odkażaniu jamy ustnej (full-mouth disinfection FMD). 9,10 Odkażanie jamy ustnej zapobiega krzyżowemu zakażeniu już oczyszczonych miejsc z nieleczonych kieszonek. Poza tym antybakteryjny efekt jest potęgowany działaniem antyseptyków. Pomimo, że efekt płukania jest znikomy w stosunku do flory poddziąsłowej, to potęguje efekt terapii eliminując patogeny z innych poza kieszonkami nisz ekologicznych. 2,10,11 Założenia FMD są oparte na dowodach o infekcyjnym pochodzeniu chorób przyzębia. Leczenie jest również korzystne z punktu widzenia pacjenta, gdyż eliminuje konieczność wielokrotnych wizyt. Quirynen i wsp. 12 sugerują, że taka terapia może być skuteczniejsza u pacjentów z niskim 373

J Stoma Do badań zakwalifikowano 20 pacjentów (11 kobiet i 9 mężczyzn) w wieku od 40 do 68 lat z umiarkowanym przewlekłym zapaleniem przyzębia. 14 U żadnego spośród badanych nie występowały inne schorzenia. Pacjenci nie byli leczeni periodontologicznie w ciągu roku poprzedzającego badania, a także nie stosowali dodatkowych środków higieny jamy ustnej (nitka dentystyczna, płyny antyseptyczne, irygatory) oraz nie przyjmowali antybiotyków 4 tygodnie przed badaniem i w trakcie trwania badania. U wszystkich badanych osób wykonano profesjonalne oczyszczenie zębów wraz z wygładzeniem powierzchni korzeni oraz instruktaż higieny jamy ustnej. Zabiegi wykonywane za pomocą narzędzi ultradźwiękowych (LM Instruments, Finlandia) i ręcznych (LM Instruments, Finlandia) trwały 60 minut. Jamę ustną odkażano płynem Listerine (Johnson&Johnson) i żelem Elugel (Pierre Fabre, Francja). Some researchers believe that there is no conclusive evidence supported by clinical, microbiological and immunological tests to prove the superiority of one-visit treatment. 13 Aim of the study The study aimed at clinical and microbiological assessment of the full-mouth disinfection protocol with own modifications by means of Elugel chlorhexidine gel and Listerine mouthwash in the initial phase of treatment of chronic periodontitis. Material and methods A total of 20 patients (11 females and 9 males) aged between 40 and 68 years with moderate chronic periodontitis qualified for the study. 14 Except for the chief complaint, the patients were healthy. None of the patients received any periodontal treatment in the past year, nor did they use any additional oral hygiene measures (floss, aseptic mouthrinses, irrigators), or take antibiotics for 4 weeks prior to or for the duration of the study. All the patients received professional prophylaxis including root planing and oral hygiene instruction. These procedures were performed by means of ultrasonic (LM Instruments, Finlandia) and manual instrumentation (LM Instruments, Finlandia) and lasted for 60 minutes. The mouth was disinfected with Listerine mouthwash (Johnson&Johnson) and Elugel gel (Pierre Fabre, France). Immediately after scaling was completed, Elugel chlorhexidine gel was applied in the pockets. As for homecare, the patients received Listerine mouthwash to rinse the mouth for two weeks (twice a day poziomem współpracy. Own modification of full-mouth disinfection protocol in the office periodontal pockets tongue saliva and mucosa oral hygiene instructions at home hygiene DISINFECTION scaling and root planing (ultrasonic and hand instruments) lasting 60 minutes subgingival irrigation with 0.2% CHX gel 0.2% CHX gel 0.2% CHX gel optimal oral hygiene rinsing twice a day with 10 ml Listerine mouthwash brushing the tongue with 0.2% CHX gel and rubbing 0.2% CHX gel into gingiva and mucosa (after rinsing) 10 ml of the liquid for 30s), and Elugel with a recommendation to brush the tongue and the buccal mucosa, and to rub the preparation into gums immediately after rinsing. Table 1 illustrates own modification of the FMD protocol. All the patients received the same kind of toothpaste to brush so as to eliminate the interference of other chemical agents. The clinical examination was performed three times with the PCPUNC 15 periodontal probe (Hu-Friedy, USA): examination I at baseline, examination II two weeks after the procedure, and examination III three months after the procedure. The following periodontal parameters were analysed: Plaque Index (PI) acc. to Silness and Löe, 15 Aproximal Plaque Index (API) acc. to Lange, 16 modified Sulcus Bleeding Index (SBI) acc. to Mühlemann and Son with Lange s modification, 16,17 Bleeding on probing (BOP) acc. to Ainamo and Bay 18 nek podawano żel chlorheksydynowy Elugel. Do stosowania w domu pacjenci otrzymywali płyn Listerine, którym płukali jamę ustną przez dwa tygodnie, dwa razy dziennie (10 ml płynu przez 30s) oraz Elugel z zaleceniem szczotkowania języka, policzków i wcierania preparatu w dziąsła bezpośrednio po płukaniu. Własną modyfikację protokołu FMD ilustruje Tab. 1. W celu wyeliminowania innych czynników chemicznych mogących wpływać na badane parametry, wszyscy pacjenci otrzymywali identyczną pastę do szczotkowania. Badanie kliniczne wykonywano trzykrotnie sondą periodontologiczną PCPUNC 15 (Hu- Friedy, USA): I badanie przed rozpoczęciem leczenia, II badanie dwa tygodnie po zabiegu, III badanie 3 miesiące po zabiegu. Oceniano następujące parametry periodontologiczne: wskaźnik płytki (Plaque Index PI) wg Silness i Löe, 15 aproksymalny wskaźnik płytki (Aproximal Plaque Index API) wg Langego, 16 zmodyfikowany wskaźnik krwawienia z kieszonek (Sulcus Bleeding Index SBI) 375
E. Dolińska et al. J Stoma Probing Pocket Depth (PPD) assessed at six sites for each tooth, Clinical Attachment Level (CAL) assessed at six sites for each tooth. During check-up examinations, 10 randomly selected patients from each group (5 females and 5 males aged between 40 and 63 years) had the material collected on each occasion from the same periodontal pocket (PPD>5 mm) for microbiological analysis. The analysis was performed by means of the Perio-Analyse set (Pierre Fabre Medicament, France). Having isolated the examined tooth from saliva, a sterile swab was used to remove supragingival plaque, and a sterile drain was placed in the pocket. After 15 seconds, the drain was removed and placed in a sterile container which was transported to the Clinident Institute lab in France. The Perio-Analyse sets enable the identification of 9 periopathogens with the realtime PCR method. They are: Aggregatibacter actinomycetemcomitans (Aa), Tannerella forsythia (Tf), Campylobacter rectus (Cr), Treponema denticola (Td), Eikenella corrodens (Ec), Prevotella intermedia (Pi), Parvimonas micra (Pm), Porphyromonas gingivalis (Pg) and Fusobacterium nucleatum (Fn). The differences between mean clinical parameters were assessed with the t-test for dependent samples. The changes in the microorganisms load in relation to time were assessed by comparing dependent variables with the Friedman s ANOVA test with Kendall s correlation coefficient in the case of many variables. The level of statistical significance was at p<0.05. For calculations, the Statistica 8.0 programme by StatSoft was used. W czasie I badania i badań kontrolnych u 10 losowo wybranych pacjentów z badanej grupy (5 kobiet oraz 5 mężczyzn w wieku od 40 do 63 lat) pobrano materiał do badania mikrobiologicznego każdorazowo z tej samej kieszonki przyzębnej (PPD>5mm). Badania mikrobiologiczne wykonywano zestawem Perio- Analyse (Pierre Fabre Medicament, Francja). Po odizolowaniu badanego zęba od dostępu śliny, jałowym wacikiem usuwano naddziąsłową płytkę nazębną i zakładano sterylny sączek w kieszonce przyzębnej. Po 15 sekundach sączek wyjmowano i umieszczano w jałowym pojemniku transportowym, który przesyłano do laboratorium Instytutu Clinident we Francji. Zestawy Perio-Analyse umożliwiają identyfikację 9 periodontopatogenów metodą real-time PCR. Są to: Aggregatibacter actinomycetemcomitans (Aa), Tannerella forsythia (Tf), Campylobacter rectus (Cr), Treponema denticola (Td), Eikenella corrodens (Ec), Prevotella intermedia (Pi), Parvimonas micra (Pm), Porphyromonas gingivalis (Pg) i Fusobacterium nucleatum (Fn). Różnice średnich badanych parametrów klinicznych zostały ocenione testem t dla prób zależnych. Zmiany liczby drobnoustrojów w czasie zbadano porównując zmienne zależne testem ANOVA Friedmana ze współczynni- 376
2011, 64, 5-6 Chronic periodontitis Zastosowany w badaniach model niechirurgicznego leczenia przewlekłego zapalenia przyzębia okazał się skuteczny i przyniósł poprawę badanych parametrów klinicznych. Uzyskano statystycznie istotne obniżenie wszystkich badanych parametrów (PI, API, SBI, BOP, PPD, CAL) zarówno po dwóch tygodniach, jak i po 3 miesiącach od badania wstępnego. Wyniki te zebrano w Tab. 2 i 3. Wskaźniki płytki nazębnej PI oraz API procentowo obniżyły się odpowiednio o 69,2% i 64,1% po trzech miesiącach. Średnia wartość wskaźnika krwawienia SBI po 3 miesiącach od badania wstępnego była o 44,2% niższa niż wartość wyjściowa. W przypadku wskaźnika BOP nastąpiła jego redukcja o 55,1%, zaś średnia głębokość kieszonek przyzębnych zmniejszyła się z 3,2 mm do 2,7 mm. The statistically significant lowering of all the selected parameters was achieved (PI, API, SBI, BOP, PPD, CAL) both after two weeks and three months of the preliminary examination. These results are presented in Tab. 2 and 3. The values of PI and API indices decreased after three months to 69.2% and 64.1%, respectively. The mean value of the SBI 3 months since the initial examination was 44.2% lower than at baseline. In the case of BOP, it was reduced by 55.1%, and the mean pocket depth decreased from 3.2 mm to 2.7 mm. Also significant improvement in the area of epithelial attachment was demonstrated 3 months after the initial examination (CAL was reduced from 3.5 to 3.0). The quantitative changes in the overall bacteria count and the overall periopathogens count are illustrated in Fig. 1. Result analysis and discussion The FMD concept was described by Quirynen et al. 9 in 1995. J Stoma two procedures of manual subgingival scaling with 24 h interval for the purpose of minimalizing transmission and translocation of periopathogens, they recommended that the following be done: brushing the tongue s dorsum for 1 min with a 1% chlorhexidine gel, rinsing the mouth twice with a 0.2% solution of chlorhexidine, subgingival irrigation of the pockets with a 1% chlorhexidine gel after each scaling and for 8 days, and rinsing the mouth with a 0.2% chlorhexidine solution twice a day for 1 min for 2 weeks. In their FMD protocol, Quirynen et al. used a rinse containing aminofluoride and stannous fluoride to prevent chlorhexidine s adverse impact. Koshy et al. 20 used 1% povidone iodine as a coolant for the ultrasonic scaler, and recommended oral rinses and tongue brushing with a 0.05% solution of chlorhexidine twice daily for a month. In this study, scaling was performed in one visit with ultrasonic and manual instrumentation, with 0.2% chlorhexidine gel application applied to gingival pockets. The patients were instructed to brush the tongue and rub Elugel preparation in the oral mucosa for two weeks after the mouth was rinsed with Listerine mouthwash. Konopka et al. 22 used Listerine mouthwash based on aromatic oils for the FMD protocol not only for rinsing but also scaling, perioirrigation and throat gurgle. They recommended brushing the tongue with 1% chlorhexidine gel. For PI, API, BOP and pocket depth assessment, they obtained statistically significant reduction of all the examined clinical parameters 3 months after treatment. Konopka evaluated the periodontal healing process after conventional non-surgical treatment and conservative treatment augmented with low doses of doxycyline. Koshy i wsp. 20 używali jako środka chłodzącego do skalera ultradźwiękowego 1% jodowanego powidonu, zalecali płukanie jamy ustnej i szczotkowanie języka 0,05% roztworem chlorheksydyny dwa razy dziennie przez miesiąc. W niniejszym badaniu skaling wykonywano w czasie jednej wizyty narzędziami ultradźwiękowymi i ręcznymi. Do dziąsłowych kieszonek podawano 0,2% żel chlorheksydynowy. Pacjentom zalecano szczotkowanie języka i wcieranie w błonę śluzową jamy ustnej preparat Elugel przez 2 tygodnie po uprzednim płukaniu jamy ustnej płynem Listerine. Płukankę na bazie olejków aromatycznych Listerine w protokole FMD stosowali Konopka i wsp. 22 nie tylko do płukania, ale także podczas skalingu, irygacji kieszonek dziąsłowych i płukania gardła. Dodatkowo zalecali szczotkowanie języka 1% żelem chlorheksydynowym. Oceniając PI, API, BOP i głębokość kieszonek przyzębnych 3 miesiące po leczeniu zyskali statystycznie istotną redukcję wszystkich badanych parametrów klinicznych. Konopka oceniał gojenie przyzębia po: tradycyjnym leczeniu niechirurgicznym i FMD, oraz po leczeniu zachowawczym wspomaganym niskimi dawkami doksycykliny. Dodatkowo analizując piśmiennictwo oceniał model gojenia tkanek przyzębia na podstawie zmiany obrazu histopatologicznego, spectrum mikrobiologicznego i wskaźników 380

He concluded that any conservative strategy utilized for the treatment of chronic periodontitis brings significant improvement of the clinical parameters, likewise the healing patterns. When analysing microbiological tests following FMD, mechanical treatment with a 24-hour interval (FMSRP) and treatment in quadrants, one has to remember that the evaluation method has a significant influence on the obtained results. 24,25 Bollen et al. 26 demonstrated statistically significant differences in favour of the FMSRP method. So did Quirynen et al. 12 who used a phasecontrast microscope in their examination. De Soete et al. 27 demonstrated the superiority of the FMSRP by means of the DNA hybridization. Conventional PCR methods did not produce statistically significant differences between the above mentioned procedural models. 20,28 They enable defining the prevalence of pathogens in a given population but cannot be used for quantitative analysis. For this study, an innovative real-time PCR method was used which enables both the qualitative and quantitative analysis of micro-organisms in the samples. It is also important to remember that not only the presence of micro-organisms in the pockets is related to the course of the disease but also their number. Real-time PCR was aslo used by Jervøe-Storm et al. 29 who reported a decrease in the bacteria count in the pockets irrespective of the applied method of treatment. Mechanical therapy reduces the plaque nonspecifically by cleansing the pocket, which is subsequently colonized by micro-organisms. It produces a change in the quality of subgingival flora. SRP is thus effective in the reduction of periopathogen count in the pocket but it klinicznych, stwierdzając, że każda strategia zachowawczego leczenia przewlekłych zapaleń przyzębia przynosi istotną poprawę parametrów klinicznych, zaś modele gojenia przebiegają podobnie. Analizując badania mikrobiologiczne wykonywane po FMD, leczeniu mechanicznym w odstępie 24 godzin (FMSRP) i leczeniu kwadrantami, należy pamiętać o tym, że duży wpływ na uzyskane wyniki ma metoda ich oceny. 24,25 Statystycznie istotne różnice przemawiające na korzyść FMSRP wykazali Bollen i wsp. 26 oraz Quirynen i wsp. 12 wykonując badania w mikroskopie fazowo-kontrastowym. De Soete i wsp. 27 za pomocą hybrydyzacji DNA potwierdzili skuteczność FMSRP. Z kolei tradycyjne metody PCR nie wykazały różnic statystycznie istotnych pomiędzy wymienionymi modelami postępowania. 20,28 Ww. metody pozwalają na określenie rozpowszechnienia patogenów w danej populacji, jednak nie dają możliwości ich oceny ilościowej. W niniejszej pracy zastosowano nowatorską metodę real-time PCR, która umożliwia oprócz oceny jakościowej także ocenę ilościową drobnoustrojów zawartych w próbce. Należy wziąć pod uwagę, że nie tylko obecność mikroorganizmów w kieszonkach patologicznych, ale także ich liczba może mieć związek z przebiegiem choroby. Real-time PCR zastosował również Jervøe-Storm i wsp. 29 oraz Swierkot i wsp. 30 i stwierdzili zmniejszenie liczby bakterii w kieszonkach dziąsłowych niezależnie od zastosowanej metody leczenia. Terapia mechaniczna redukuje płytkę w sposób niespecyficzny poprzez oczyszczenie kieszonki, po czym następuje jej ponowne zasiedlenie przez drobnoustroje. Powoduje to zmianę w jakości flory poddziąsłowej. SRP jest więc skuteczny w redukcji liczby periopatogenów w kieszonce, nie wpływa jednak na obecność tych bakterii w innych miejscach 381
E. Dolińska et al. J Stoma does not affect the presence of these bacteria elsewhere in the mouth. 31 It is, therefore, justifiable to administer antiseptics to reduce the risk of re-infection. Wennström et al. 33 compared the cost of initial FMSRP periodontal treatment performed by means of ultrasonic instrumentation in one hour with conventional manual procedures in quadrants (approximately 3 hrs). The number of pockets with PPD 4mm three months after FMSRP was 58%, and after conventional manual procedures in quadrants 66%. The effectiveness of the initial periodontal treatment evaluated as a relation of the duration of treatment to the number of pockets with PPD 4mm was statistically significantly higher for FMSRP (3.3 min per one pocket with PPD 4mm) at 8.8 min. The European Periodontal Workshops which were held in 2008 also signal the need to check the effect of different antiseptics and the 24-hour therapy on the periodontal condition in patients with chronic periodontitis. The guidelines set out during the Workshops indicate the need to assess the impact of various mechanical therapy protocols on the periodontium, with emphasis on their cost. 34 It was also observed that the three models of treatment (treatment in quadrants, FMSRP and FMD) are appropriate, and their choice depends on the experience of the dental practitioner and the patient s needs. Conclusion On the basis of clinical and microbiological results obtained in this study it can be concluded that our own modification of the FMD model is the effective method of treatment, and can be utilized in the initial phase of treatment of chronic periodontitis. jamy ustnej. 31 Stąd zasadne jest stosowanie antyseptyków w celu zmniejszenia ryzyka reinfekcji. Wennström i wsp. 33 porównywali koszty wstępnego leczenia periodontologicznego FMSRP wykonanego narzędziami ultradźwiękowymi w czasie ograniczonym do jednej godziny z typowym leczeniem narzędziami ręcznymi kwadrantami (łącznie ok. 3 godzin). Procentowa liczba kieszonek z PPD 4mm 3 miesiące po FMSRP wyniosła 58%, zaś po leczeniu narzędziami ręcznymi kwadrantami 66%. Efektywność wstępnego leczenia periodontologicznego mierzona jako stosunek czasu leczenia do liczby kieszonek z PPD 4mm była statystycznie istotnie wyższa dla FMSRP (3,3 min. na jedną kieszonkę z PPD 4mm) 8,8 min. Europejskie Warsztaty Periodontologiczne, które odbyły się w 2008 roku także kierują uwagę badaczy na konieczność sprawdzania wpływu różnych antyseptyków, jak i samej terapii 24-godzinnej na stan przyzębia u pacjentów z przewlekłym zapaleniem przyzębia. W wytycznych z warsztatów sugeruje się konieczność oceny wpływu różnych protokołów terapii mechanicznej na stan przyzębia, ze zwróceniem uwagi na ich koszt. 34 Zwrócono także uwagę, że wszystkie trzy modele postępowania (leczenie kwadrantami, FMSRP i FMD) są prawidłowe i wybór metody powinien zależeć od doświadczenia lekarza dentysty oraz potrzeb pacjenta. Podsumowanie Na podstawie uzyskanych wyników z badania klinicznego i mikrobiologicznego można stwierdzić, że własna modyfikacja modelu FMD jest skuteczną metodą postępowania i może być stosowana we wstępnej fazie leczenia przewlekłych zapaleń przyzębia. 382
