4. Streszczenie w języku polskim 4. Streszczenie w języku polskim Ekspresja wybranych markerów komórkowych w nowotworach mastocytarnych skóry u psów. Badania przeprowadzono łącznie na 105 guzach z komórek tucznych skóry u psów, zabezpieczonych w formie bloczków parafinowych pochodzących z archiwum Katedry Patologii Wydziału Medycyny Weterynaryjnej UPWr. Skrawki parafinowe zostały zabarwione standardową metodą z użyciem hematoksyliny i eozyny (H&E) oraz dodatkowo błękitem toluidyny w celu uwidocznienia ziarnistości cytoplazmatycznych. Ponadto przeprowadzano reakcje immunohistochemiczne (IHC) z użyciem przeciwciał skierowanych przeciwko: 1) CD117 (receptor czynnika wzrostowego komórek tucznych) 2) markerom proliferacji komórek, takich jak: a. p16 b. Ki-67 c. PCNA (proliferating cell nuclear antigen) d. MCM-3 (mini-chromosome maintenance protein) 3) markerom komórek śródbłonka naczyń krwionośnych, takich jak: a. FVIII (czynnik von Willebranda) b. VEGF (Vascular Endothelial Growth Factor) c. LIMS-1 (LIM and senescent cell antigen-like-containing domain protein 1) d. CD31 Podczas oceny ekspresji białka CD117 wyróżniano 3 typy reakcji barwnej: pierwszy występował jako silny odczyn błonowy oraz nieznaczne zabarwienie cytoplazmy, drugi charakteryzował się intensywnym, ale ogniskowym wybarwianiem cytoplazmy, a trzeci - 15 -
4. Streszczenie w języku polskim przedstawiał rozlane, delikatne zabarwienie obejmujące całą cytoplazmę, w szczególności okolicę jądra komórkowego. Ekspresję wymienionych w pkt. 2 markerów proliferacji komórek oceniono metodą półilościową na podstawie ilości wybarwionych jąder komórkowych w 5 losowo wybranych polach widzenia: - 0-5% - brak reakcji [-], - 6-25% - słaba reakcja [+], - 26-50% - średnia reakcja [++], - powyżej 50% - intensywna reakcja [+++]. Ekspresję markerów śródbłonka naczyń krwionośnych określono na podstawie 5 najintensywniej unaczynionych obszarów, w których zliczano ilość wybarwionych naczyń krwionośnych pod powiększeniem 200x. Pierwszy etap badań obejmował analizę 45 zmian. Nowotwory zabarwiono metodą H&E oraz błękitem toluidyny i po analizie mikroskopowej podzielono guzy zgodnie ze skalą wg Patnaik i wsp., gdzie: 11 z nich stanowiły MCT I, 21 MCT II i 13 MCT III. Następnie wykonano badania immunohistochemiczne z użyciem białek: CD117, p16 oraz FVIII. Białko CD117 wykazywało, w zależności od stopnia złośliwości guza, różne typy reakcji barwnej. W 100% guzów MCT I występował pierwszy typ wybarwiania; w MCT II 47,6% zmian charakteryzowało się drugim wzorem reakcji barwnej; wszystkie nowotwory zdiagnozowane jako MCT III wykazywały trzeci typ reakcji. Współczynnik korelacji rho Spearmana pomiędzy ekspresją CD117, a stopniem złośliwości dla p<0.001 wynosił 0,85. Wyniki te wskazały na silną zależność pomiędzy typem obserwowanej reakcji, a stopniem mastocytoma. Analiza ekspresji FVIII oraz p16 wykazały również ścisły związek ze złośliwością histologiczną guza z komórek tucznych. Badania statystyczne potwierdziły silną korelację pomiędzy tymi białkami, a stopniem mastocytoma. Współczynnik korelacji rho Spearmana dla p<0.001 wynosił odpowiednio 0,9 (dla FVIII) oraz 0,84 (dla p16). Uzyskane wyniki pozwoliły na stwierdzenie, że w guzach z komórek tucznych ekspresja wszystkich badanych markerów wzrastała wraz ze stopniem złośliwości. Przedstawione wyniki zostały opublikowane w pracy pt Useful immunohistochemical indicators in canine mast cell tumours w Acta Veterinaria Hungarica. - 16 -

Streszczenie w języku polskim W drugim etapie badań 60 nowotworów podzielono zgodnie ze skalą wg Patnaik i wsp., następnie wykonano badania immunohistochemiczne z wykorzystaniem: CD31, LIMS- 1 oraz VEGF. Zgodnie z klasyfikacją wg Patnaik i wsp. 16 zmian zaliczono do grupy MCT I, 26 MCT II i 18 MCT III. W badaniach stwierdzono brak korelacji pomiędzy stopniem złośliwości, a ekspresją VEGF (p 0.05). Podczas analizy ekspresji CD31 testem rang Spearmana (p 0.05), również nie stwierdzono korelacji ze stopniem złośliwości guza. Jednak testem ANOVA rang Kruskala-Wallisa wykazano, że pomimo braku istotnej korelacji pomiędzy stopniem złośliwości guza, a ekspresją białka, obserwuje się statystycznie silniejszą (p<0.05) ekspresję w MCT II i III, niż w I. Dodatnią, słabą korelację stwierdzono pomiędzy stopniem złośliwości mastocytoma, a ekspresją białka LIMS-1, współczynnik korelacji rho Spearmana był równy 0,34 dla p<0.05. Ponadto testem ANOVA rang Kruskala-Wallisa wykazano, że więcej naczyń krwionośnych wybarwiono w MCT II i III, niż w MCT I. Uzyskane wyniki wykazały, że jedynie białko LIMS-1 może być wykorzystywany jako skuteczny marker neoangiogenezy w guzach typu mastocytoma. Uzyskane wyniki zostały opublikowane w Bulletin of the Veterinary Institute in Pulawy w pracy Immunohistochemical evaluation of neoangiogenesis in canine mast cell tumours. Podczas trzeciego etapu badań również 60 guzów podzielono zgodnie ze skalą wg Patnaik i wsp. oraz Kiupel i wsp., a następnie poddano analizie immunohistochemicznej uwzględniającej: Ki-67, MCM-3 oraz PCNA. Zgodnie ze skalą wg Patnaik i wsp. 16 zakwalifikowano jako MCT I, 26 MCT II i 18 MCT III ; wg klasyfikacji wg Kiupel i wsp. 28 guzów o niskiej złośliwości i 32 o wysokiej złośliwości. Podczas oceny ekspresji PCNA obserwowano pozytywną reakcję w większości guzów (59 przypadków - 98,3%). Niezależnie od użytej skali oceny nowotworów stwierdzono bardzo intensywną reakcję prawie we wszystkich zmianach. Analiza statyczna testem rang Spearmana wykazała istotną statystycznie (p<0.05) dodatnią korelację pomiędzy stopniem złośliwości w barwieniu H&E, a PCNA, która wynosiła dla skali oceny wg Patnaik i wsp. 0,6, a wg skali Kiupel i wsp. 0,53. W badaniach z użyciem Ki-67 stwierdzono silną zależność (p<0.05) ze stopniem złośliwości ocenionym w barwieniu H&E zarówno zgodnie ze skalą wg Patnaik i wsp. (0,63), jak i wg Kiupel i wsp. (0,85). Analiza ekspresji MCM-3 również wykazała silną korelację pomiędzy ekspresją białka, a złośliwością histologiczną ocenioną dla obu badanych skal, współczynnik korelacji rho Spearmana wynosił 0,67. - 17 -
4. Streszczenie w języku polskim Przeprowadzona analiza statystyczna wykazała, że wszystkie użyte białka proliferacji komórek mogą być stosowane w mastocytoma. Z uwagi na bardzo silną reakcję PCNA, co wiąże się prawdopodobnie z obecnością tego białka również w komórkach, które już zakończyły podział mitotyczny, nie jest ono zalecane do rutynowej diagnostyki. Dodatkowo wykonano ocenę tempa proliferacji komórek na podstawie analizy indeksu mitotycznego. Szczegółowe wyniki zostały zawarte w pracy Comparative analysis of markers of cell proliferation in canine mast cell tumours according to current classifications w Polish Journal of Veterinary Sciences. Streszczenie w języku angielskim The expression of selected cell markers in canine skin mast cell tumours The study was conducted on a total of 105 canine skin mast cell tumours, which were preserved in the form of paraffin blocks and archived at the Department of Pathology of the Faculty of Veterinary Medicine at the Wroclaw University of Environmental and Life Sciences. The paraffin-embedded sections were stained with hematoxylin (H&E) as well as toluidine blue to visualize cytoplasmic granules. In addition, immunohistochemistry (IHC) was performed using the following antibodies: 1) CD117 (mast cell growth factor receptor) 2) Markers of cell proliferation, including: a. p16 b. Ki-67 c. PCNA (proliferating cell nuclear antigen) d. MCM-3 (mini-chromosome maintenance protein) 3) Markers of endothelial cells of blood vessels, such as: a. FVIII (von Willebrand factor) b. VEGF (Vascular Endothelial Growth Factor) c. LIMS-1 (LIM and senescent cell antigen-like-containing domain protein 1) d. CD31 Three types of colour reactions were distinguished when assessing the expression of the CD117 protein. There was strong membrane staining and slight cytoplasmic staining in the first group and intense focal cytoplasmic staining in the second group. Diffuse, delicate staining of the whole cytoplasm, particularly visible in the area of the nucleus, was characteristic in the third group. The expression of the markers of cell proliferation listed above was assessed semiquantitively based on the amount of stained cell nuclei in 5 randomly selected areas: - 0-5% - no reaction [-], - 19 -
5. Streszczenie w języku angielskim - 6-25% - weak reaction [+], - 26-50% - moderate reaction [++], - over 50% - intense reaction [+++]. The expression of the markers of endothelial cells of blood vessels was assessed on the basis of the five most intensely vascularised areas. In those areas, the number of stained blood vessels were counted at a 200x magnification. In the first part of the study, 45 lesions were assessed. The tumour specimens were stained using H&E and toluidine blue. They were then analysed by microscopy and divided into three groups according to the scale by Patnaik et al. Eleven specimens were graded as MCT I, 21 were MCT II and 13 were MCT III. CD117, p16 and FVIII (the von Willebrand factor) were detected using immunohistochemistry. The CD117 protein showed different colour reactions depending on the malignancy of the tumour. First type of IHC staining was visible in 100% of the MCT I tumours. In 47.6% MCT II tumours a second type of colour reaction was observed, and all MCT III tumours presented the third type of staining. The rho Spearman s correlation between CD117 and the degree of malignancy was 0.85 at p<0.001. These results pointed to a strong correlation between the type of reaction observed and the grade of the mastocytoma. The analysis of the FVIII and p16 expression also showed a close relationship with the histological malignancy of the mast cell tumour. The statistical analysis confirmed a strong correlation between these proteins and the grade of mastocytoma. The rho Spearman s correlation at p<0.001 was 0.9 for FVIII and 0.84 for p16. On the basis of the results, we were able to conclude that the expression of all the studied markers increases with the degree of malignancy of the mast cell tumour. The results, titled Useful immunohistochemical indicators in canine mast cell tumours, were published in Acta Veterinaria Hungarica. In the second part of the study, 60 tumours were assessed based on the scale by Patnaik et al. Sixteen specimens were graded as MCT I, 26 as MCT II and 18 as MCT III. Immunohistochemistry was used to detect CD31, LIMS-1 and VEGF expression. There was no correlation between the degree of malignancy of the tumour and the expression of VEGF (p 0.05). Similarly, no correlation was found using the Spearman s rank correlation test between the expression of CD31 and the malignancy of the tumour (p 0.05). However, despite no statistically significant correlations between the grade of the tumour and - 20 -
5. Streszczenie w języku angielskim the CD31 expression, there was a statistically significant (p<0.05) difference in the expression of this protein between MCT II and III groups than in I ones. A weak positive correlation was found between the degree of malignancy of the mast cell tumours and the expression of LIMS-1, whereby the rho Spearman s correlation coefficient was 0.34 at p<0.05. Moreover, an analysis using the Kruskal-Wallis ANOVA test indicated that more blood vessels were presented in MCT II and III tumours than MCT I ones. These results suggest that the LIMS-1 protein can be the only protein used as an effective marker of angiogenesis in mast cell tumours. The results were published in the Bulletin of the Veterinary Institute in Pulawy under the title of Immunohistochemical evaluation of neoangiogenesis in canine mast cell tumours. In the third part of the study, 60 specimens were graded according to the scale by Patnaik et al. and Kiupel et al. and an immunohistochemical analysis was carried out to detect Ki-67, MCM-3 and PCNA. According to the scale by Patnaik et al., 16 tumours were classified as MCT I, 26 as MCT II and 18 as MCT III. Twenty eight tumours were classified as low grade and 32 as high grade according to the scale by Kiupel et al. A positive reaction was observed in the majority of the tumours during the evaluation of the PCNA expression (59 cases 98.3%). Regardless of the scale used to grade the tumours, a strong reaction was observed in almost all the specimens. A statistically significant positive correlation between the degree of malignancy seen in the H&E staining and PCNA (p<0.05) was found using the Spearman s rank correlation test, which amounted to 0.6 when using the scale by Patnaik et al., and 0.53 when using the scale by Kiupel et al. A strong correlation (p<0.05) was also found between the expression of Ki-67 and MCT grade determined with either the Patnaik (0.63) or the Kiupel (0.85) system. A strong correlation was also found between the expression of MCM-3 and the histological malignancy of the tumour assessed using both scales, where the rho Spearman correlation was 0.67. The statistical analysis suggested that all markers of cell proliferation can be used to assess the mast cell tumours. Owing to the strong reaction of PCNA, most likely caused by the presence of this protein in cells that have completed mitosis, this protein is not recommended for use in routine diagnostics. Additionally, the rate of cell proliferation was analysed based on the mitotic index.
