STRESZCZENIE Rany przewlekłe stanowią poważny problem dla lekarzy różnych specjalizacji jak i chorych, prowadzając do obniżenia jakości ich życia. Problem ten dotyka rocznie około 20 milionów ludzi na całym świecie, w Polsce 500 tysięcy, a ich liczba stale wzrasta. Do najczęstszych ran przewlekłych należą owrzodzenia żylne goleni, które występują u 1,0 2% dorosłej populacji krajów uprzemysłowionych. W leczeniu owrzodzeń żylnych goleni w zależności od stanu rany i fazy gojenia stosuje się opatrunki tradycyjne lub nowoczesne. Coraz częściej stosuje się również opatrunki z miodu Manuka. Celem pracy była ocena wpływu opatrunku z miodu Manuka na proces gojenia przewlekłych owrzodzeń pochodzenia żylnego. Materiał badawczy stanowiła dokumentacja medyczna 80 chorych z przewlekłymi owrzodzeniami żylnymi goleni leczonych w latach 2013-2015 w Poradni Chirurgicznej Szpitala Specjalistycznego nr 2 w Bytomiu, Przychodni Chirurgicznej PCZ S.A. w Kluczborku, Przychodni Chirurgicznej przy Klinice Medycyny Paliatywnej Hospicjum Stacjonarnego UM w Poznaniu. Metoda: Analiza retrospektywna obejmowała: płeć i wiek chorych, charakter pracy, klasyfikacja niewydolności żylnej CEAP, wskaźnik kostka-ramię API, wskaźnik BMI, okres od wystąpienia owrzodzenia, powierzchnia owrzodzenia, kolorowa skala oceny rany, wymaz z rany, nasilenie dolegliwości bólowych rany. Badani zostali podzieleni na dwie grupy, każda po 40 osób. U chorych z grupy 1 zastosowano opatrunek z miodu Manuka Activon Tulle, w grupie 2 maść Argosulfan produkcji Polfa. U wszystkich chorych po założeniu odpowiedniego opatrunku zastosowano kompresjoterapię. Wszyscy badani przed rozpoczęciem leczenia, a następnie co 7 dni, mieli wykonywany planimetryczny pomiar powierzchni owrzodzenia, aż do czasu wygojenia owrzodzenia. U wszystkich badanych przed podjęciem leczenia, jak również po 3 i 5 tygodniach terapii, wykonywano badanie mikrobiologiczne. Wyniki: U wszystkich chorych z grupy 1 owrzodzenia zagoiły się w ciągu 7 tygodni leczenia. Natomiast u chorych w grupie 2 w ciągu tego samego czasu trwania leczenia wygoiło się zaledwie dwanaście ran. Pozostałe owrzodzenia wygoiły się w ciągu 14 tygodni stosowania opatrunków z maści Argosulfan. Analiza flory bakteryjnej wykazała, iż wszystkie rany przed rozpoczęciem leczenia u chorych w obu grupach były zakażone. Wszystkie owrzodzenia u badanych w grupie 1 leczone opatrunkiem z miodu Manuka nie zawierały drobnoustrojów.
Wnioski: Opatrunek z miodu Manuka wpływa na zmniejszenie powierzchni owrzodzeń żylnych goleni i tym samym skraca czas leczenia. Zastosowanie opatrunku z miodu Manuka powoduje szybsze usunięcie flory bakteryjnej z owrzodzeń goleni pochodzenia żylnego. Słowa kluczowe: owrzodzenie żylne goleni, opatrunki, miód Manuka.
ABSTRACT Needless to say, chronic wounds are said to constitute a serious problem both for doctors of various specialisations as well as patients, since it leads to the significant deterioration of their quality of life. The problem affects roughly 20 million people around the world, with 500 cases in Poland where the number is constantly increasing. The most common chronic wounds constitute venous ulcers, found in 1,0 2% of adult population in the industrialized countries. In the treatment of venous ulcers, traditional as well as modern dressings are widely applied, depending on the condition as well as the phase of the healing process. Yet, more and more commonly used proves to be the Manuka honey dressings. The main aim of the thesis was to assess the influence of the Manuka honey dressing on the process of chronic wounds healing. The material: consisted of medical records of 80 patients, who suffered from venous ulcer and who underwent the medical treatment between 2013-2015 in the Outpatient Surgery Center No. 2, Specialist Hospital No. 2, Batorego 15, 41-902 Bytom, Poland, Outpatient Surgery, Chair and Department of Palliative Medicine, Surgery Clinic for out-patient, Hospice Palium, Os. Rusa 25A, University of Medical Science, 61-848 Poznań, Poland Outpatient Surgery, NZOZ District Hospital, Skłodowskiej-Curie 23, 46-200 Kluczbork, Poland The method: the retrospective analysis included: the patients sex, age, type of work, CEAP, API and BMI indicators, the initial appearance as well as the surface of the ulcer, the so called colorful scale of the wound assessment, the swab, let alone the intensity of pain. The subjects were divided into two groups, 40 patients respectively. The treatment in the first group involved the application of the Manuka Activon Honey dressing, whereas in the case of the second group, the Argosulfan ointment produced by Polfa was used. After the application of the above mentioned dressings, all the patients underwent the compression therapy. Additionally, before the commencement of the treatment, and then every 7 days, the patients had the planimetric measure of the ulcer surface taken, until the full completion of the healing process, let alone the microbiological examination performed before the treatment as well as after three and five weeks of the applied therapy. The results: The healing process of the ulcer was reported to be completed within the period of 7 weeks, after the commencement of the treatment in the first group of patients. However,
in the case of the second group of patients, after the 7 weeks of treatment, the completed healing process was reported merely in the case of 20 patients. The healing process in the remaining cases was completed within 14 weeks of the applied treatment with the use of Argosulfan ointment. Furthermore, the bacterial flora analysis revealed that all the wounds were infected before the commencement of the treatment. Of worth mentioning is the fact that the ulcers in the patients from the first group, who were treated with the application of the Manuka honey dressing, did not contain any germs. Concluding remarks: The Manuka honey dressing exerts a noticeable influence on the reduction of the venous ulcer surface, simultaneously leading to the reduction of the treatment time. The application of Manuka honey brings about significantly swifter reduction of the bacterial flora which is likely to be found in the venous ulcers. Key words: venous ulcers, dressings, Manuka honey
