Aim of the study: To determine the content of metals in deciduous teeth of children both exposed and not exposed to tobacco smoke. Material and methods: Deciduous teeth collected during their physiological replacement with permanent dentition were examined. The content of elements (Mn, Fe, Cu, Ca, K, Na, Mg, Zn, Cr, Cd, Pb) was determined by means of the AAS method. Results: Deciduous teeth of children who were not exposed to tobacco smoke had a higher content of elements whose physiological role for the organism is recognized (Fe, Zn, K, Na, Ca) than deciduous teeth of children exposed to passive smoking. Conclusion: The research has shown higher levels of toxic metals, which may come from tobacco smoke, in hard tissues of the teeth of children exposed at home to passive smoking. Streszczenie Wstęp: w warunkach narażenia na dym tytoniowy zawarte w nim metale toksyczne zostają włączane w strukturę zmineralizowanych tkanek organizmu, np. zębów. Zwiększone stężenie szkodliwych pierwiastków może wywierać wpływ na zawartość pierwiastków fizjologicznie niezbędnych dla organizmu. Cel pracy: badano zawartość metali w zębach mlecznych dzieci narażonych i nienarażonych na dym tytoniowy. Materiał i metody: badano zęby mleczne dzieci uzyskiwane w wyniku fizjologicznej wymiany uzębienia. W zębach oznaczono pierwiastki Mn, Fe, Cu, Ca, K, Na, Mg, Zn, Cr, Cd, Pb metodą AAS. Wyniki: w zębach mlecznych dzieci, które nie były narażone na dym tytoniowy, stwierdzono większą zawartość pierwiastków o uznanej roli fizjologicznej dla organizmu (Fe, Zn, K, Na, Ca), w porównaniu do zawartości tych metali w zębach mlecznych dzieci narażonych na dym tytoniowy. Wnioski: wskazano na większy poziom zawartości metali toksycznych, których źródłem może być dym tytoniowy, w zmineralizowanych tkankach zębów dzieci narażonych na bierne palenie. 2011, 64, 8 Passive smoking Introduction Not only smokers, but also people living in their environment, are exposed to toxic substances contained in tobacco smoke. Nonsmokers are exposed to environmental tobacco smoke (ETS). Toxic substances contained in tobacco smoke migrate with air circulation, and then through inhalation penetrate the human body. During exposure to tobacco smoke, the mineral composition of selected tissues of the organism may be modified. Toxic metals incorporated into the structure of hard tissues, such as bones or teeth, during the process of mineralization are retained for a long time. Exposure to toxic metals, which are a component of tobacco smoke, increases their content in hard tissues and affects the content of elements that are physiologically necessary for the organism. 1-3 The final form of the metals content in hydroxyapatites of deciduous teeth is a combination of behavioral factors (place of residence, passive smoking), biological factors (gender, type of tooth and its position in the mouth), as well as environmental factors (secondary industrial and transport emissions, the content of elements in dust suspended in the air in a given area). Passive smoking is toxic, especially for children. Children are very sensitive to toxic substances, which also affect the overall physiological balance of their organisms. Due to the fact that deciduous teeth constitute a relatively easily accessible biological material collected during exfoliation, they can be used as a bioindicator of exposure to metals. 4-6 Aim of the study The study was conducted to determine the content of metals in primary teeth: Mn, Fe, Wstęp Na działanie szkodliwych substancji zawartych w dymie tytoniowym narażona jest nie tylko osoba paląca, lecz również osoby przebywające w jej otoczeniu. Osoby niepalące są eksponowane na tzw. środowiskowy dym tytoniowy (Environmental Tobacco Smoking ETS). Substancje toksyczne zawarte w dymie tytoniowym przemieszczają się wraz z powietrzem, a następnie poprzez drogi oddechowe przedostają się do organizmu człowieka. W warunkach narażenia na dym tytoniowy modyfikacjom może podlegać skład mineralny tkanek organizmu. Metale toksyczne włączane podczas procesu mineralizacji w strukturę twardych tkanek, tj. kości czy zęby są tam zatrzymywane. Narażenie na metale toksyczne, zawarte w dymie tytoniowym, powoduje ich zwiększenie w tkankach zmineralizowanych, a także wywiera wpływ na zawartość pierwiastków fizjologicznie niezbędnych dla organizmu. 1-3 Ukształtowanie się w sposób ostateczny zawartości metali w hydroksyapatytach zębów jest wypadkową wpływów czynników behawioralnych (miejsce zamieszkania, bierne palenie), czynników biologicznych (płeć, rodzaj zęba oraz jego położenie w jamie ustnej), a także czynników środowiskowych (wtórna emisja pyłów przemysłowych i komunikacyjnych, zawartość pierwiastków w pyle zawieszonym w powietrzu na danym obszarze). Na skutki biernego palenia szczególnie narażone są dzieci. Organizm dziecka jest bardzo wrażliwy na działanie substancji toksycznych, które wpływają także na ogólną równowagę fizjologiczną. Ze względu na to, że zęby mleczne są stosunkowo łatwo dostępnym materiałem biologicznym, pozyskiwanym w czasie naturalnej wymiany uzębienia mlecznego na stałe, mogą być wykorzystywane jako bioindykator ekspozycji na metale. 4-6 621
A. Fischer et al. J Stoma Cu, Ca, K, Na, Mg, Cr, Zn, Cd and Pb in terms of exposure to tobacco smoke from cigarette smoking by parents in the presence of children. Material and methods The material consisted of deciduous teeth of children aged 7-13 years old and living in the province of Silesia since birth. The teeth were collected from children at the time of physiological replacement with permanent dentition. From each child, a whole tooth (incisors and canines without carious lesions) was obtained for the study. A confidential questionnaire on the child s place of birth and residence, the child s age, general health, and information on the exposure to tobacco smoke at home was filled in by children s carers and attached to the sample. Children in whose presence at least 10 cigarettes were smoked daily qualified as those exposed to passive smoking. Children who had no contact with tobacco smoke at home were categorized as those non-exposed to passive smoking. The study was conducted on 69 deciduous teeth (38 from non-exposed children and 31 from those exposed to ETS). The study was conducted with the approval of the Bioethics Committee of SMU No NN-6501-109/06. Collected teeth were subject to laboratory procedure. The teeth were thoroughly washed and rinsed with distilled water. Then they were dried to ensure complete dryness (80⁰C/ 24h). The next step involved the process of mineralization using nitric acid (Merck, suprapure). 7,8 The determination of metals (Mn, Fe, Cu, Ca, K, Na, Mg, Cr, Zn, Cd and Pb) in the teeth was carried out with the AAS method (Philips Pye Unicam SP- 9) in acetylene-air flame. The verification of accuracy was established by means of the Cel pracy Celem pracy była ocena występowania metali: Mn, Fe, Cu, Ca, K, Na, Mg, Cr, Zn, Cd i Pb w zębach mlecznych w aspekcie narażenia na dym tytoniowy, którego źródłem było palenie papierosów przez rodziców. Materiał i metody Materiałem badawczym były zęby mleczne dzieci w wieku od 7 do 13 lat, zamieszkujących od urodzenia na terenie woj. śląskiego. Zęby mleczne były pozyskiwane od dzieci w czasie fizjologicznej wymiany uzębienia mlecznego na stałe. Od każdego dziecka do badań przekazywany był jeden cały ząb. Do badań przeznaczano zęby sieczne oraz kły bez zmian próchnicowych. Do zębów dołączano wypełnioną przez opiekunów dzieci poufną ankietę obejmującą następujące dane: miejsce zamieszkania i urodzenia dziecka, wiek dziecka, ogólny stan zdrowia, a także informacje na temat narażenia dzieci w warunkach domowych na dym tytoniowy. Za dzieci narażone na bierne palenie uważano te, w obecności których wypalano codziennie minimum 10 papierosów. Dzieci nienarażone na bierne palenie nie miały w ogóle kontaktu z dymem tytoniowym w rodzinnym domu. Przedmiotem badań było 69 zębów mlecznych, spośród których 38 pochodziło od dzieci nienarażonych, zaś 31 od dzieci narażonych na bierne palenie. Badania wykonano za zgodą Komisji Bioetycznej SUM nr NN-6501-109/06. Zebrane zęby mleczne były przekazywane do badań laboratoryjnych. Po umyciu, przepłukaniu wodą destylowaną i wysuszeniu do stałej masy (80⁰C/24h) zęby były poddawane procesowi mineralizacji na mokro za pomocą kwasu azotowego (Merck, suprapure). 7, 8 Oznaczanie 622
2011, 64, 8 Passive smoking standard additional method using the model of mass concentration of given metals (produced by Division of Physics and Chemistry of the Head Office of Measures in Warsaw). The procedure was also validated by NIST 1486 (Bone Meal). Statistical analysis of the results was based on the Statistica program for Windows ver. 8.0 pl. Mann-Whitney U test was used to evaluate the statistical significance of the data. Application of cluster analysis enabled the grouping of metals in relation to their concentrations in teeth. Ward s method was used for data agglomeration and dimensionless Euclidean distance was assumed as a unit of measure. Results Table 1 presents the mean values of metals with known physiological role for the body (Mn, Fe, Cu, Ca, K, Na, Mg, Zn and Cr) and toxic metals (Cd and Pb) in deciduous teeth of children exposed and non-exposed to passive smoking at home. Comparative analysis revealed that the concentration of physiological elements (Mn, Fe, Cu, Cr, Zn, Ca, K, Na) was statistically insignificantly higher in deciduous teeth of children not exposed to passive smoking in comparison with the teeth exposed to passive smoking. The only exception was magnesium whose content significantly higher in teeth of children exposed to smoking (p 0,05). The concentration of toxic metals (Pb and Cd) was higher in children exposed to passive smoking. At the level of probability p=0.05 the difference was statistically insignificant. In order to observe the interactions occurring between the studied metals the correlation analysis was performed. Deciduous teeth of children not exposed to passive smoking revealed more directly proportional significant changes in the content of the studied metals zawartości Mn, Fe, Cu, Ca, K, Na, Mg, Cr, Zn, Cd i Pb w próbkach wykonywano metodą AAS z użyciem aparatu Phillips Pye Unicam SP-9 w płomieniu acetylen-powietrze. Poprawność oznaczeń sprawdzano metodą dodatku wzorca z wykorzystaniem standardowych stężeń odpowiednich metali (prod. Główny Urząd Miar, Zakład Fizykochemii, Warszawa), a także poprzez oznaczenia metali w materiale referencyjnym NIST-1486 (Bone Meal). Analizę statystyczną otrzymanych wyników wykonano w programie Statistica for Windows wersja 8.0 pl. Do oceny znamienności statystycznej pomiędzy badanymi grupami zastosowano test U Manna-Whitneya. W analizie statystycznej otrzymanych wyników zastosowano analizę skupień, pozwalającą na uporządkowanie metali ze względu na ich zawartość w zębach. W analizie zastosowano aglomerację metodą Warda, a za miarę odległości przyjęto bezwymiarową odległość euklidesową. Wyniki Średnie ilości poszczególnych metali o znanej roli fizjologicznej dla organizmu (Mn, Fe, Cu, Ca, K, Na, Mg, Zn i Cr) oraz metali toksycznych (Cd i Pb) w zmineralizowanych tkankach zębów mlecznych dzieci narażonych i nienarażonych w warunkach domowych na dym tytoniowy zestawiono w Tab. 1. Porównanie zawartości metali wykazało, że w zębach dzieci nienarażonych na bierne palenie w warunkach domowych zaobserwowano nieznacznie większe poziomy metali fizjologicznie występujących w organizmie (Mn, Fe, Cu, Cr, Zn, Ca, K, Na), w porównaniu do zębów dzieci narażonych na bierne palenie. Różnice te nie były istotne statystycznie. W zębach dzieci narażonych na bierne palenie stwierdzono znamienny statystycznie (p 0,05) większy 623

(Tab. 2). Their occurrence is illustrated by the results of cluster analysis Fig. 1 and 2. The method by Ward was used for data agglomeration and Euclidean distance was the assumed measure of distance. The location of metals in dendrograms presented explicit information on their levels in primary teeth while the analysis of the distance of the bonds between individual metals revealed differences between the two investigated populations. Discussion The study revealed a slightly higher content of metals with known physiological role for the body (Mn, Fe, Cu, Ca, K, Na, Mg, Zn and Cr) in deciduous teeth of children not exposed to passive smoking in comparison with the teeth of children exposed to passive smoking at home. The amount of potassium in the teeth of both groups of children was similar at 119 µg/g. In the teeth of children exposed to passive smoking the levels of toxic metals (Pb and Cd) were higher. However, at the poziom Mg. Ilość metali toksycznych (Pb i Cd) była większa w zębach dzieci narażonych na dym tytoniowy. Na poziomie prawdopodobieństwa p=0,05 różnice te nie były statystycznie istotne. W celu zanalizowania interakcji pomiędzy badanymi metalami w zębach mlecznych dzieci narażonych i nienarażonych na bierne palenie wykonano analizę korelacji. Zęby mleczne dzieci nienarażonych na bierne palenie charakteryzowały się większą liczbą wprost proporcjonalnych istotnych zmian zawartości badanych metali (Tab. 2). Sposób występowania badanych metali w zębach mlecznych ilustrują wyniki analizy skupień Fig. 1 i 2. W analizie zastosowano aglomerację metodą Warda, zaś za miarę odległości przyjęto odległość euklidesową. Położenie metali względem siebie w otrzymanych wykresach hierarchicznych daje przejrzystą informację o ich poziomie w zębach mlecznych, a analiza odległości wiązania między poszczególnymi metalami wykazuje zróżnicowanie badanych grup. 626
2011, 64, 8 Passive smoking Uzyskane wyniki badań wykazały nieznacznie większą zawartość metali fizjologicznie niezbędnych dla organizmu (Mn, Fe, Cu, Cr, Zn, Ca, Na) w zębach dzieci nienarażonych na bierne palenie w porównaniu do zębów dzieci narażonych na bierne palenie w warunkach domowych. Ilość potasu w zębach dzieci narażonych i nienarażonych na bierne palenie była zbliżona i wynosiła 119 µg/g. W zębach dzieci narażonych na bierne palenie zanotowano większy poziom metali toksycznych dla organizmu Pb i Cd, jednakże na poziomie prawdopodobieństwa p=0,05 różnice nie były statystycznie istotne. Spadek w zębach ilości metali fizjologicznych dla organizmu i wzrost stężenia metali toksycznych, których źródłem jest dym tytoniowy jest zgodny z doniesieniami z piśmiennictwa i wskazuje, że dym tytoniowy może być dodatkowym źródłem metali toksycznych w organizmie, a także wywiera wpływ na zawartość pierwiastków fizjologicznie niezbędnych dla organizmu. 2,9-11 Niniejsze badania wykazały także zwiększenie minimalnej ilości niektórych metali fizjologicznie niezbędnych, jak np. żelaza, wapnia sodu i magnezu w zębach dzieci narażonych w warunkach domowych na działanie dymu tytoniowego. Fakt ten może sugerować, że w tkankach zmineralizowanych pod wpływem działania dymu tytoniowego może dochodzić nie tylko do zwiększenia ilości pierwiastków toksycznych, ale do kompensacyjnego zwiększenia zawartości metali fizjologicznie tam występujących. Analiza skupień uzyskanych wyników badań wykazała, że zarówno w zębach dzieci nienarażonych jak i narażonych na bierne palenie, badane pierwiastki tworzyły podobne grupy występowania w zębach mlecznych. Tobacco smoke may be an additional source of toxic metals in the body and affecting the content of elements necessary for the body. 2,9-11 This study also showed an increase of the minimal content of some physiologically essential metals (Fe, Ca, Na and Mg) in the teeth of children exposed to tobacco smoke at home. This fact may suggest that in the hard tissues there may occur not only an increase of the content of toxic elements, but also the compensatory increase in content of metal physiologically present there. Cluster analysis of results revealed that both in the teeth of children exposed and not exposed to passive smoking the studied elements occurred in similar groups in deciduous teeth. The differences were only reflected in the change in dimensionless Euclidean distance binding. In the teeth of children not exposed to passive smoking, the Euclidean distance was longer than in the teeth of children exposed to passive smoking. This change in the Euclidean distance binding indicates a decrease in diversity of content of analyzed elements in the teeth of children exposed to passive smoking (Fig. 1, 2). Correlation analysis revealed that deciduous teeth of children not exposed to passive smoking had a higher number of directly proportional significant changes in the content of the metals in question in comparison with teeth exposed to passive smoking (Tab. 2). Directly proportional co-occurrence between Pb and Cd was characteristic of dentition of both populations. A. Fischer et al. J Stoma Na and Mn, Pb and Zn in the teeth of children not exposed, and between Na and Mn, Cd and Cu in the teeth of children exposed to passive smoking. It is remarkable that for the exposed population of children, the inversely proportional relationship was observed for the co-existence of sodium and cadmium, while in the teeth of children not exposed, the sodium content varied inversely to the Pb content (Tab. 2). In the case of magnesium whose content was statistically significantly higher in the teeth of children exposed to passive smoking, the change of content of this element was significantly correlated only with the change of the Cu content. Significant correlations between concentrations of Mg and Pb, Cd, Zn and Fe, present in the teeth of children not exposed to passive smoking, suggest a close interaction between Mg and toxic elements. Such a relationship does not occur in children exposed to cigarette smoke, which may indicate that the hard tissue of teeth of children exposed to passive smoking is less sensitive to changes in the content of toxic elements, due to the occurrence of compensatory mechanisms taking place during exposure. Conclusion 1. The results of this study showed a statistically insignificant increase in the content of toxic metals (Pb, Cd) in the mineralized tissues of the teeth of children exposed to passive smoking at home. 2. The content of physiologically essential elements in the deciduous teeth of children not exposed to passive smoking was slightly higher when compared to the population exposed to passive smoking. The differences were not statistically significant. tali znalazły swoje odzwierciedlenie jedynie w zmianie bezwymiarowej wielkości odległości euklidesowej wiązania. W zębach dzieci nienarażonych na bierne palenie stwierdzono wydłużenie odległości euklidesowej w porównaniu do zębów dzieci narażonych na bierne palenie. Ta zmiana odległości wiązania wskazuje na zmniejszenie zróżnicowania zawartości badanych pierwiastków w zębach dzieci narażonych na bierne palenie (Fig. 1, 2). Wyniki analizy korelacyjnej wykazały, że zęby mleczne dzieci nienarażonych na bierne palenie odznaczały się większą liczbą wprost proporcjonalnych istotnych zmian zawartości badanych metali w porównaniu z zębami dzieci narażonych na bierne palenie (Tab. 2). Współwystępowanie o charakterze wprost proporcjonalnym pomiędzy Pb i Cd charakteryzowało zarówno zęby dzieci narażonych jak i nienarażonych na bierne palenie. Odwrotnie proporcjonalne, istotne współzależności zaobserwowano pomiędzy Na i Mn, Pb i Zn w zębach dzieci nienarażonych oraz pomiędzy Na i Mn, Cd i Cu w zębach dzieci narażonych na bierne palenie. Interesującym jest fakt, że w przypadku dzieci narażonych na dym tytoniowy, odwrotnie proporcjonalna zależność charakteryzowała współwystępowanie sodu i kadmu, podczas gdy w zębach dzieci nienarażonych zawartość sodu zmieniała się odwrotnie do zawartości toksycznego dla organizmu Pb (Tab. 2). W przypadku magnezu, którego zawartość była statystycznie istotnie większa w zębach dzieci narażonych na dym tytoniowy zmiana ilości tego pierwiastka była istotnie skorelowana jedynie ze zmianą ilości Cu. Istotne korelacje pomiędzy ilością Mg a Pb, Cd, Zn i Fe występujące w zębach dzieci nienarażonych na bierne palenie wskazują na ścisłe oddziaływanie pomiędzy Mg i pierwiastkami szkodliwymi. Zależności takie nie występują u dzieci narażonych na dym tytoniowy, 628

Wnioski 1. Wyniki badań wykazały statystycznie nieistotne zwiększenie ilości metali toksycznych (Pb, Cd) w zmineralizowanych tkankach zębów dzieci narażonych w warunkach domowych na dym tytoniowy. 2. Ilość pierwiastków fizjologicznie niezbędnych w zębach mlecznych dzieci nienarażonych w warunkach domowych na bierne palenie była nieznacznie wyższa w porównaniu do narażonych na dym tytoniowy. Zaobserwowane różnice nie były statystycznie istotne. 3.
