Learning Agreement 1. Choose courses (ask your coordinator about the subjects in english or other foreign language available at UKSW in the 2014/2015 academic year. Ask your mentor for help with choosing the proper subjects. Note, that you can choose courses from the whole offer of courses in foreign languages available at UKSW. 2. Contact your faculty coordinator at UKSW for approval; 3. Bring completed LA Changes form to IRO; 4. If you do not want to change anything in your Learning Agreement put No changes on the Changes form and do everything what is mentioned in the point 2. and 3.

Registration for courses After making changes to LA you have to registrate for the chosen courses. Registration for courses is held in the Dean s Office. Without registration you will not get Examination Card, ergo you will not get the Transcript of Records.
Examination Card This is a document that you collect all your marks and teachers signatures on after each exam. Take it from the Dean s Office, before your 1 st exam; Collect marks, and signatures after each exam; Give it to your Faculty Coordinator to approve and sign; Bring completed examination card to IRO before departure; IRO will prepare TRANSCRIPT OF RECORDS (TR) for you.
Polish language course Polish language course is full-paid by student. Cost of the course: 40 participants: 570 pln/person 30 participants: 690 pln/person 20 participants: 950 pln/person 15 participants: 800 pln/person Participation in the course gives you an opportunity to earn 1,5 ECTS point.
Legalize of your staying in Poland EU/EFTA citizens can come over to Poland with no visa and stay on the teritory of Poland with no legalization for 3 months. After this time they should register their stay in Poland. Not EU/EFTA citizens should get visa before departure to Poland.
