37 Investment activities Investment activities to be implemented within this project will be based on: reconstruction of the building belonging to Polkowice Medical and Healthcare Centre ZOZ S.A. in order to create a Center for Gerontology Care and Support of The Dependent People,

38 creation of a computer network (connection of the the created Center for Gerontology Care with the internal network and medical data system of Polkowice Medical and Healthcare system and with the system ewuś) purchasing medical equipment and any necessary equipment. The implementation of these tasks is an essential condition to extend the scope of services provided to the elderly and dependents and increase the quality of medical care.

39 Non- investment activities health and education services Factual activities within the project were divided into four areas 1. SENIOR - psychological rehabilitation program 2. physical rehabilitation program 3. program of patient care at home 4. education and counselling

40 SENIOR - psychological rehabilitation program individual and group psychotherapy; psychoeducation (autogenous training, lectures, training sessions); psychological diagnostics (cognitive tests in which the psychotechnical and neuropsychological tests will be carried out );

41 neuropsychological therapy; consultations for participants and families in the field of addictions; occupational therapy (art therapy - music therapy, cultural - educationalactivities, singing group); speech therapy; Consultation of a specialist psychiatrist; psychotechnical tests for drivers over 60 years of age.

42 Physical rehabilitation program 1. Outpatient rehabilitation carried out in the the Outpatient Clinic 2. Post- hospital Rehabilitation Emergency primary home visits Sudden deterioration of health condition with limited physical ability arouses among patients and their families a lot of fears and concerns "what happens next".

43 So called primary home visits of patients over 60 years of age and for dependent people will be started. Home visits are aimed at: educating the family and patient - interview providing the educational materials conducting instruction proceedings informing about how to further proceed with the patient

44 begin and prepare for rehabilitation improvement; establish contact with Centre for Gerontology Care.

45 3. Home rehabilitation care of the patient carried out as a part of a comprehensive program of a patient care at home.

46 Program of a patient care at home: The third area of activity is a program of a patient care at home carried out by nurses, doctors and support staff. It is addressed mainly to elderly patients and chronically and terminally ill, in need of assistance in the field of palliative and long- term care. Activities for these patients aim at soothing their pain and other symptoms as well as psychological, social and spiritual support. The aim is to improve the quality of patient s life.

47 Education and counselling The last area of the project activities is education and counseling to patients, their families and medical staff. The project will provide numerous training courses for these groups of people in the scope of care of the elderly and dependent. In addition, educational materials will be distributed among them through radio broadcasts, medical journals, medical and social networking sites, as well as leaflets, booklets and instructions.

48 Tele- counselling An important part of this project is TELE- COUNSELLING - a new service, which task is to increase the sense of security of sick and disabled people staying at home. Thanks to a TELE- COUNSELLING service it will be possible to obtain medical information and use medical care and psychological support to the extent possible to be provided on the phone nonstop.

49 Rehabilitation Equipment Hire We will begin the rehabilitation equipment hire which will serve patients and their carers, especially in the first period of dependence before the family or other aid institutions provide the patient permanentlywith these facilities.

50 Beneficiaries of the project 1. direct beneficiary: Polkowice Medical and Healthcare Centre - ZOZ S.A. 2. final beneficiaries: a group of people who are directly involved in the project patients people qualified for the project; medical staff of 110 people: doctors, nurses, physiotherapists, occupational therapists, psychologists, speech therapists who will improve the skills in advanced care therapies for the elderly and dependent;

51 Non- organizational organizations members of 35 people involved in the support of the elderly as well as social workers and carers of elderly people who will participate in training and information sessions as a part of the project; families and other people connected with the patients, whose numbers are estimated at approx. 700 people will also benefit from the results of the project
